“It’s as if you’re not there, as if you’re hovering outside your body and playing on autopilot. It probably only happens four or five times in your career.”
The record-breaker on ‘batting nirvana’.

@goldstone_tony @elcarthy Tony, thanks for the great videos! I'd love an explainer for those of us who have taken a bit more of a long-winded route to consultancy & have contributions in the 2008 section? For e.g. Who or what is McCloud?! If useful, scheme info attached & am 37 yo & 1 year into cons job
Rachel Reeves: "The cost to the taxpayer of Covid fraud is estimated at £7.2bn. Every cheque signed by Rishi Sunak...We'll appoint a Covid corruption commissioner, a hit squad of investigators...to take the fraudsters to court...We want that money back."
I would also like to remind you that Boris Johnson was the PM that introduced voter ID in 2022. In May 2024 he was turned away from a polling station for not bringing…voter ID. Another example of Tory incompetence and arrogance
I would like to remind you of the untold damage inflicted to the UK by Liz Truss & Kwasi Kwarteng. Incompetence & arrogance . They have set back this waning country by decades. When we ask why labour do not have ambitious policies, look at them. hands are tied by their legacy.
Sooooo....we're now doing gastric POCUS for ALL GLP-1 agonist pts. And the results are...not pretty. It changes our mgmt frequently. Rather than rely on an arbitrary fasting time, a bedside test has been key. If you're new to this, check out this vid 👇

@bhanders@cliffreid@SydneyHEMS @MedSTAR_SA And I worry about the ergonomics of the NIV. If vent starts alarming you are then looking at/fiddlling with the vent and not looking at patient and not managing their airway - which is likely the root cause of the problem. (E.g. airway is not patent or there’s a big leak etc)
@bhanders@cliffreid@SydneyHEMS @MedSTAR_SA I’d prefer to manually bag with a mapleson C/waters when apnoeic. Plus can support spont ventilation with pos pressure pre drug delivery if needed. Gives tactile feedback about leak and patency of airway when you are delivering pos pressure through bag.
