Here are some tips for diuresing patients🧵
Furosemide - backbone of diuresis. Start at a higher than you think and back off if diuresis achieved.
Metolazone (or other thiazides). Use when Na on the higher side as helps prevent distal Na resorption. You get more naturesis.
Acetazolamide. Use to prevent metabolic alkalosis. Aids in decongestion.
Spironolactone. Use to prevent K+ wasting and to achieve more natriuresis.
3% saline. Consider in diuretic resistant heart failure with hypochloremia.

OK, in general a "less is more" approach is reasonable & there is a "rationale" behind many of the following but the truth is that they don't make sense if scrutinized

Firm pressure (20-30 lbs) over 1-2 finger tips area (0.15-0.3 in²) applies ~100-130 psi of pressure.
That’s slightly more than a semi trucks tires apply to the road: 90-120 psi.
This is why direct pressure works! You can literally push harder than a truck!
Everyone should know that one of the most effective ways to prevent someone from bleeding out is by simply applying DIRECT PRESSURE. Hold firm pressure as directly onto the wound as possible. Do NOT use excess gauze or towels as this just distributes the pressure over a larger surface area. If you do this right, you can literally save someone’s life with just one finger!
