Yes. There are some organizations doing the things you're alleging. They were wrong before & remain so. These are also generally large organizations who employ a lot of clinical staff - PT factories.

There are SO MANY pieces to be assembled before something like that can be rolled out. Not making excuses for CMS, but there needs to be policy + support (procedure). I believe we’ll see permanent changes (rolling back would require similar massive change)
