@hjluks Lateral closing wedge osteotomy at malunion site, IM nail fixation with nail slightly anterior and countersunk, subsequent TKA with trabecular metal tibia that avoids nail altogether
Probably because of demographics / demand in South Florida, I went from 100% trauma when I first started 15 y ago to now 90% total joints + revisions with the remainder pelvic / periarticular “cold” trauma. Easier to do in a non-academic environment where less “turf” issues in the department…
@OneGSisneros Great thread - and the “good” that is promised is defined in verse 29, so that we can be “conformed to the image of His Son”, i.e. sanctification, not an earthly benefit or resolution to our trial
Articulating spacer whenever possible, but in cases of significant bone loss, will use a static spacer with knee-spanning tibial nail. Freehand locked proximally and distally to facilitate full-weightbearing - haven’t had issues with stress riser or cement extrusion on reimplantation
Meet Teresa! After years of hip pain, she trusted Dr. Daniel Chan, Chief of Orthopaedic Surgery & Sports Medicine at Memorial, for a life-changing hip replacement.
Walking the next day, back to work in 2 weeks, now she’s enjoying pain-free pickleball with her son! 🎾💪💙
@hjluks You don’t need a joints fellowship to be able to do a decent primary hip or knee - fellowships are for getting good at complex cases / revisions.
@ArthroToday@JennaBernstein7@Dr_Stambough@Ortho_Deck@AAHKS Thanks for the tag - I personally think having surgery done in a hospital is the biggest risk factor for SNF discharge. When surgery is done in an ASC and SNF isn’t an option, there is much more emphasis on pre-op optimization and setting up social support / home services

@generalorthomd@OrthoBroPA Solid work - reminds me of a residency mentor who used to say that the most inconvenient or difficult thing for you is probably the right thing to do for the patient.

"My only regret is not doing this sooner." Maria endured 13 months of pain from osteoarthritis. Dr. Chan's approach spared major muscles, ensuring a swift recovery. Just 2 days post-surgery, Maria was celebrating the new year, bidding farewell to the limitations she once had.
@hjluks “If you wouldn’t be proud to tell your mother or your residency program director about it, you probably shouldn’t do it” - I first heard this in reference to docs who are in the pocket of personal injury attorneys but I feel it applies here as well.
