Week 4 of #AcuteCVDays tackles a fascinating and increasingly relevant concept: “The inflamed heart.”💓
Not simply myocarditis. Not simply pericarditis. But an entire inflammatory cardiovascular continuum.
This matters because modern acute cardiovascular care is increasingly intersecting with:
• Autoimmune disease
• Systemic inflammation
• Immune checkpoint inhibitor toxicity
• Viral syndromes
• Post infectious inflammatory responses
• Inflammatory cardiomyopathies.
The heart is not an isolated pump. It is an immunologically active organ.
And inflammation changes everything:
• Arrhythmia risk
• Ventricular function
• Hemodynamics
• Thrombosis
• Long term remodeling
• Sudden cardiac death risk
One particularly important teaching point from this week: Endomyocardial biopsy is no longer viewed as an “academic only” tool.
In selected high risk presentations such as fulminant myocarditis, malignant arrhythmias, or rapidly progressive heart failure, tissue diagnosis may directly influence treatment decisions including immunosuppressive therapy.
Meanwhile, cardiac MRI has transformed non invasive diagnosis and risk stratification, particularly in stable patients with infarct like presentations and unobstructed coronaries.
The future of acute cardiac care may increasingly depend on combining: Imaging.
Immunology.
Molecular profiling.
Hemodynamics..
And precision phenotyping.
This is exactly why inflammatory cardiovascular syndromes are becoming central topics in cardiovascular intensive care education.

Fulminant myocarditis may evolve within hours from:
• Mild chest pain
• Preserved LVEF
• Modest troponin elevation
…to refractory cardiogenic shock requiring:
• High dose vasopressors
• VA ECMO
• Mechanical ventilation
• Temporary pacing
• Mechanical circulatory support, and yet early diagnosis remains difficult because presentations are extraordinarily heterogeneous.
This week’s ACVC material highlights a critical concept: Myocarditis is not one disease. It is a spectrum.
Viral injury.
Immune mediated inflammation. Autoimmune overlap.
Toxic triggers.
Genetic susceptibility.
Inflammatory cardiomyopathies.
The real challenge is identifying which patient will deteriorate.
That is why contemporary management increasingly integrates:
• Clinical phenotype
• Arrhythmic burden
• Biomarkers
• Echocardiography
• Cardiac MRI
• Selective endomyocardial biopsy
• Hemodynamic profiling
The old paradigm viewed myocarditis mainly as a diagnosis of exclusion.
The modern paradigm treats it as an active inflammatory cardiovascular syndrome requiring early risk stratification, and perhaps the most important question: Are we recognizing fulminant myocarditis before circulatory collapse occurs?
