One minute after takeoff from Juanda International Airport in Surabaya, Indonesia, the first officer (FO) of an Airbus A320 noticed that the captain was unresponsive. The FO of the Citilink Indonesia flight conducted an emergency return to Surabaya, where the captain was pronounced dead. None of the 171 passengers were injured.

During the takeoff roll, the pilots of the Fairchild SA227 Metroliner noticed the unusually slow increase in airspeed. Because they were uncertain whether there was enough runway remaining to safely stop, they continued the takeoff, then realized that the covers were still on the pitot tubes.
