There are many technical reasons for these types of failure, but there are two structural ones
1. Monopolies build what is easiest for the monopoly and not for the consumer
2. Government agencies routinely ignore governance issues related to application performance

Also, no governance layer existed to own and manage performance on the project. This doomed the project from the outset to fail under load
@AliZeeshan01 Root cause, developers over allocated resources per session. Reducing your size, duration of hold, and frequency of request will reduce your origina server load per session, allowing for a higher load for your finite resource pool
