Axillary dissection for low-volume nodal involvement afterneoadjuvant therapy in breast cancer: multicentre AXSANA cohortstudy

🎗️ Completion axillary lymph node dissection (cALND) is often performed after neoadjuvant chemotherapy (NACT) when low-volume disease is found in the sentinel or target lymph node.
📊 In the international prospective AXSANA cohort (2020–2024), outcomes of cALND were analysed in patients with ITCs or micrometastases after NACT.
🔍 cALND led to nodal upstaging in a small subset (from ypN1mi to ypN2–3), indicating additional nodal disease.
🧪 Importantly, none of the upstaged patients achieved pCR in the breast, and nodal upstaging did not alter post-NACT treatment decisions.
❌ These findings suggest that cALND adds little clinical value for decision-making in low-volume nodal disease after NACT and should not be used for diagnostic purposes alone.
