Community Health Centre #CHC serving residents of the South Riverdale community since 1976. Improving health, contributing to a healthy, inclusive community.
What if healthcare were organized around the people and neighbourhoods it serves?
As we celebrate 50 years and Community Health and Wellbeing Month, we’re #DreamingForward with Dr. Catherine Yu, family physician and co-founder of Health Access Taylor-Massey — a neighbourhood-based model bringing primary care, community and social services together around local residents.
Her dream?
“Instead of asking people to navigate the system, bring the health and community system together around them.”
That’s a future worth building.

Dreaming Forward ☁️➡️
As we celebrate 50 years of care, connection, and community, we’re asking people across our community to share their hopes for the next 50.
This Community Health and Wellbeing Month, we’re listening to Jason Miles, Harm Reduction and Lived Experience Advocate.
His dream is rooted in his own journey — and in the people who helped him find a way forward:
“I dream of a future where nobody feels like they’re too far gone to come back. Where everyone has a home, someone who believes in them, and a chance to build a life that has meaning. I had people at SRCHC who kept showing up for me when I couldn’t see a way forward. I want other people to have that same chance.”
As we honour 50 years of SRCHC, we’re dreaming forward toward a future where everyone has that chance.
#DreamingForward #50YearsofCare #CommunityHealthMonth
For 50 years, SRCHC has shown what’s possible when healthcare meets people where they are — with compassion, respect, and trust.
This Community Health and Wellbeing Month, we’re celebrating the people and ideas that help carry that approach forward.
During a two-week practicum with our Mental Health & Substance Use program, Hanna Müller, a Masters of Nursing student from Germany, experienced that approach firsthand. Now, she’s taking what she learned at SRCHC back home.
Her dream for the next 50 years:
“My dream for SRCHC is that it continues to grow without ever losing what makes it special — the trust, compassion, and respect at the heart of its work. I hope more people can experience the kind of care I saw here, where they are met where they are and supported to make their own choices. I want to take that experience back to Germany and help create more possibilities for community-based care there, too.”
That’s the power of dreaming forward — imagining what’s possible, then carrying it with us.

Before there was a building, there was a belief: health begins with community, everyone
deserves care with dignity, and ordinary people can create extraordinary change. The
South Riverdale Community Health Centre was shaped not by one founder, but by
many — community organizers, health care providers, volunteers, advocates, and
champions who imagined a different kind of health care and worked together to make it
real.
This October, as we continue to celebrate our 50th anniversary and mark Community Health and Wellbeing Month, we’re honouring the visionaries who helped shape South Riverdale Community Health Centre from its very beginnings.
Through these profiles of our founding voices, we hear their reflections, revisit the early days of SRCHC, and celebrate the remarkable legacy they helped create — a legacy that continues to shape our community and inspire our work today.
We’re proud to share these stories and honour the people whose vision helped make SRCHC what it is today.

It’s Community Health and Wellbeing Month and we’re celebrating the power of community and care — and 50 years of serving our community — by sharing dreams for the future from the people who make SRCHC so special.
"Our dream is for SRCHC to continue growing alongside our communities so that every person, no matter their background or life experience, feels welcomed, supported, and connected. We dream of more opportunities for people to discover they are not alone, to find hope, friendship and belonging, and to access programs that reflect the diversity of our community. We dream of dancing forward together — with compassion, celebration, and care — for the next 50 years and beyond."

In celebration of our 50th anniversary, and in recognition of Community Health and Wellbeing Month beginning this Thursday, we’re sharing the next set of our 50 Years, 50 Faces profiles — shining a light on some of the amazing people who make our community what it is.
For 50 years, SRCHC has been built on the belief that care is about more than health. It’s about connection, belonging, and community — and the people who show up for one another in ways both big and small.
Today meet harm reduction advocate Jason Miles, family physician Dr. Catherine Yu, nursing student Hanna Müller, reception team staff Idy Lee, and CAMH Chief Medical Officer Sanjeev Sockalingam. Read their stories here: https://t.co/bOsVe4Lk8a

For 50 years, we’ve believed that care starts by seeing the whole person — and that we’re stronger when we build it together.
Our Primary CONNECT partnership with CAMH and Parkdale Queen West Community Health Centre is rooted in that belief. Together, we’re helping create pathways to primary care for people who have too often faced barriers to finding a health home.
As we Dream Forward, Sanjeev Sockalingam, CAMH’s Chief Medical Officer, reminds us what’s possible:
“The future is about building care around people’s lives — not asking people to fit themselves into our systems.”
Here’s to the next 50 years of dreaming, building, and caring — together.

50 years of community health care. And we're just getting started.
And with that, we’re proud to share our 2026-2031 Strategic Plan: Creating a Future Where Everyone Belongs.
Looking ahead, our new Strategic Plan charts the next chapter of our work: advancing health equity, strengthening community connections, continuing to adapt care to meet emerging needs, and creating a culture of belonging, learning, and growth for our staff.
As we celebrate 50 years of impact, we're focused on the future and the work still ahead.

As we celebrate 50 years of caring for our community, we're dreaming forward together.
Health care is about meeting people where they are, with care, compassion, and connection — because health is shaped by so much more than what happens in an exam room.
Dreaming Forward:
"We dream of a future where every person is connected to primary care, and where everyone's basic needs are met, so everyone has the opportunity to live a healthier life and thrive." — Our Integrated Primary Connect Team

For 50 years, we've stood alongside people facing some of life's biggest challenges — with compassion, respect, and care.
As we celebrate our anniversary, one of our Community Health Workers shares her dream for the future:
"Dreaming forward means making sure every person feels seen, valued, and supported — wherever they are on their journey." — Clare Tumwebaze
That's the future we're working toward, together.

Last Saturday, we hosted a community clean-up event, in celebration of our 50th anniversary! ♻️
Together, we were able to collect the litter and waste in our neighbourhood. This initiative allowed us to give back to the community and work towards having a cleaner, healthier, more sustainable environment. 🌎
Thank you to all the volunteers and community members who contributed their time and effort to this community clean-up. 💚
Here’s to the next 50 years of making a difference together! 🎉

Yesterday, South Riverdale Community Health Centre, our partner organizations, and the community, came together for International Overdose Awareness Day. We took the time at this event to honour the lives of those we have lost to overdose, provide support to those impacted by the overdose crisis, and share vital resources.
Thank you to everyone that joined us and showed their support and to all of our community partner organizations that participated in this event.
We appreciate the support and this event was a great reminder that we are stronger when we stand together!💜

Dreaming Forward with Rocky Dobey
As part of our 50th anniversary, we’re celebrating people whose stories, creativity and vision have helped shape our community.
Rocky is the Toronto artist behind our Memorial for People Who Use Drugs. His art honours lives, creates space for remembrance and calls us to imagine a more caring future.
As we mark International Overdose Awareness Day, we’re also gathering with community partners to remember, connect and look toward a future rooted in care.
Join us Monday, Aug. 31, 11 a.m.–3 p.m. at Moss Park for food, activities, and opportunities to connect with local organizations.
So, what does Rocky dream for SRCHC’s next 50 years?
“I dream of SRCHC having the resources to offer even more social services and more space, including another building specifically for housing. Supportive housing and social services are so important. Less and less resources are being put toward social services, and that needs to change. Right now.”
That’s a dream we can get behind.

Dreaming Forward: 50 years of community care.
As we celebrate 50 years of caring for and alongside our community, we’re looking ahead through the dreams of the people and partners who help shape our work.
For Patrick Esenerwa, Case Manager with TNG Community Services and part of our East Toronto HART Hub, harm reduction is about supporting people in the ways they need to move forward — through housing, employment, connection, practical supports, and finding purpose. As an artist, he also sees the healing power of creativity.
“Even creating art is a form of harm reduction.”
Join us for International Overdose Awareness Day, Monday, Aug. 31, 11 a.m.–3 p.m. at Moss Park, alongside our community partners.
