I also have a 5090 and have encountered a similar problem. I’ve used both the official and modified clients and they still have the issue.
In the NVIDIA App, I enabled Image Scaling through Global Settings, and it works exactly as intended when playing games in fullscreen or borderless. However, it stopped upscaling everything the moment I enabled Discord overlay, and it hooked on the application/game.
Considering Discord is just WebView, that might be the issue. Although, Chrome also properly upscales when it’s maximized. So does Steam Overlay. In all honesty, I don’t know, and wouldn’t know if this is related to the issue OP mentioned. Figured I’d mention this just in case.

Long detailed explanation, bare with me
Back then, Windows only had one projector. You can only put one sheet on its screen to display at any given time. Whether it was a game you were playing, or watching a video, you can only do one or the other. Attempting to put two sheets— playing a video and a game at the same time— on this projector will only display the first sheet that was placed first sometimes. Other times, it’ll overlap on top of each other, causing what’s being projected to become garbled— visual artifacts and glitches.
You decide to take both sheets and cycle them on the projector. You put one of the sheets on for 5 seconds, before taking it off and putting the other sheet on for 5 seconds, and repeating the process. Technically you’re displaying both sheets on the same projector, but this is inefficient and disorientating— flickering.
MPO allows the GPU to barge in the classroom with 4 projectors or more depending on the card, and have them on standby. Each one of these projectors have their own power cable and line up perfectly with one another. So if you want to display three sheets at once, you’re able to do that since you have three individual projectors to do so.
It’s why watching a video, having a second monitor and playing a game at the same time works flawlessly.
Discord is forcing Windows to act like it only has the one projector. Because of that, it’s causing it to do that sheet swapping cycling method-thing.
