Without that visibility, states are measuring effort, not outcomes.

The Unite Us team is heading to the 2026 #Maryland @StateofReform conference on May 21st! We’re ready to dive into the most pressing issues facing Maryland’s healthcare system today.

At this year's conference, we’re proud to contribute to the conversation on 𝘤𝘰𝘮𝘱𝘭𝘦𝘹 𝘥𝘪𝘴𝘤𝘩𝘢𝘳𝘨𝘦 𝘤𝘰𝘰𝘳𝘥𝘪𝘯𝘢𝘵𝘪𝘰𝘯. Our Director of Policy, Natalie Daniel, will share insights on how integrated tools support care teams in moving patients safely from the hospital back into their communities.

Natalie will be joining leaders from Johns Hopkins Bayview Medical Center (@HopkinsMedicine), University of Maryland Medical Center (@umms), Keswick, and Health Management Associates (@HMAConsultants) to exchange insights and drive progress on these longstanding challenges.
