🔵 Socket Preservation Most of the alveolar ridge resorption occurs within the first 3–4 months after extraction, with significant reduction in ridge width, especially on the buccal aspect. 🔵🔹what is Socket Preservation? Socket preservation is a procedure performed immediately after tooth extraction to reduce bone loss and help maintain the ridge dimensions. 🔵🔹When do we need it? • Future implant placement is planned • Thin buccal bone is present • Esthetic zone cases • Large extraction sockets • When we want to preserve bone volume for prosthetic stability 🔵🔹How is it done? After atraumatic extraction: 1️⃣ The socket is cleaned carefully 2️⃣ A graft material or scaffold is placed inside the socket 3️⃣ A membrane may be used if needed 4️⃣ The site is then sutured for healing Studies showed that socket preservation may help reduce horizontal ridge resorption compared to spontaneous healing, although some dimensional changes still occur during healing. 📌 Palatal Torus: A Small Anatomical Feature That Can Make Dentures Fail Not every denture challenge comes from poor technique. Sometimes, it’s anatomy. A palatal torus is a bony prominence that appears along the midline of the palate. Its size and shape vary greatly from person to person, and it is found in approximately 10% of the population, occurring more frequently in women than in men. The larger the torus, the greater the challenge for denture stability. In mild cases, a complete denture can often be fabricated without surgery. In more severe cases, surgical removal by an oral surgeon may be considered before treatment. Why does it matter? The mucosa covering the palatal midline is often very thin. When a prominent torus is left untreated, the denture may use this area as a fulcrum. ➡️ Functional pressure on one side can lift the opposite side. ➡️ Denture retention decreases. ➡️ Painful pressure spots may develop. For these patients, relief design becomes critical. After impression making, relief may not be necessary if the residual ridge and supporting tissues are favorable. However, when ridge support is compromised or the mucosa is fragile, approximately 1 mm of relief over the torus can help distribute forces more appropriately and improve patient comfort. She had 31 additional teeth, a rare condition known as multiple hyperdontia. While this condition is often associated with genetic syndromes, comprehensive testing in her case did not reveal any underlying syndrome or genetic disorder. The extra teeth were crowded and irregularly shaped, creating challenges for normal oral development, chewing function, and dental alignment. Managing such a complex condition requires a multidisciplinary approach involving oral surgeons, orthodontists, and prosthodontists to carefully plan treatment and restore proper function and appearance.
