USMLE Tip: Metformin can cause lactic acidosis by blocking conversion of lactate to glucose. This is a high yield fact.

Phenformin, a similar drug in the same class, was pulled from the market in the 1970s due to severe cases of lactic acidosis. With metformin, the frequency is much lower and occurs almost exclusively in patients with renal failure or other serious illness (shock, hypoxia, etc). This is why we hold metformin in many acutely ill hospitalized patients. We can't be sure their renal function and general status will remain stable.
