Drain management after pancreatoduodenectomy: risk-stratified dynamic algorithm

This multicentre study of 788 patients undergoing pancreatoduodenectomy (PD) evaluates the reliability of drain fluid amylase (DFA) as a predictor of postoperative pancreatic fistula among high-risk (HR) and non-HR-PD. We found that, whereas early drain removal guided by DFA and serum enzymes is safe in non-HR-PD, DFA accuracy is reduced in HR-PD until postoperative day 5, when combined DFA and C-reactive protein levels allow safe drain removal. These findings support a dynamic, risk-based drain management protocol tailored to pancreatic fistula risk.

Postpancreatectomy diarrhoea affects over one-third of patients despite appropriate enzyme replacement, significantly impairing quality of life. This prospective study identified key risk factors for moderate-to-severe diarrhoea, including vascular resection, arterial divestment, and pancreatic ductal adenocarcinoma aetiology. The study findings highlight the need for personalized postoperative management to mitigate the clinical and therapeutic impact of postpancreatectomy diarrhoea.
