Q9: With the increasing use of biologics in allergic diseases, do you think it is possible that some of the patients we are treating with biologics never meet criteria for HES because we are treating them before they would have met criteria? Why or why not?

The effects of corticosteroids can linger for a few weeks. If an eosinophilic disorder is suspected & it is safe to stop corticosteroids, an eosinophil count can be checked again after a few weeks to see if there is an elevated baseline level.

Eosinophil counts quickly drop (often to zero) after oral/iv corticosteroids. Do NOT use a normal eosinophil count to rule out eosinophilic disease without verifying a person was not on corticosteroids.

Great point @winder_gill! The differential for eosinophilia is huge. Allergist-immunologists are always happy to help but also appreciate if you are also considering non-allergic causes 😀

@AIMedEdJC I think there is a larger 'takehome' message not just for us, but for #primarycare and those involved with our patients - We often see eosinophilia on labwork, and yes, it can be associated with atopic disorders, but please consider the differentials! #AIMedEdJc@AIMedEdJC

@shaunah07@PAallergy@AIMedEdJC If you can make the eos go down some with skin care...then more likely AD associated eosinophilia. If no matter what you do to the skin eos remain elevated and refractory skin dz then probably other way around. No easy answer but clinical judgement #AIMedEdJC
