问：66岁，女，肺腺癌3期靠后，19年10月做过左肺上叶切除，后一直在吃靶向药。新冠疫情期间接种3剂疫苗。今年4月23日复查加强CT，显示腹部有2个团状物，5月13日完善pet/ct检查结果确认肺部转移至腹膜及妇科附件。抽血CEA指数1993；彩超检查提示双侧胸腔积液腹腔积液，部位：胸腹水+心包5/27病理检查检查报告提示胸水细胞块包埋细胞学诊断见少量恶性细胞，酶标提示肺腺癌。医生告知考虑化疗等待通知。目前吃饭没胃口，胸闷，体重明显下降近10斤，腹部除腹胀不透气外，偶尔有钝痛。期待得到您的指导。

答：这个患者病史听上去比较复杂。首先，老年本身肺腺癌7年前已经做了手术，但是可能2021年以来接种了三剂毒疫苗，然后现在等于说是全身转移了。你看，他复查胸腔积液已经发现肺腺癌，那就是原先的肺腺癌又复发了，并且多器官转移。你看转移到胸腹腔都有，因为他积液细胞检查都有恶性细胞，而且胸腹腔都有积水，恶病质出现了；因为体重严重下降，而且出现——因为他积液——胸闷、腹胀、腹腔积液，这些症状都出现。根据这个患者的病史和各项检查结果分析，这个癌种已经不再局限于肺部，远处转移、胸腹腔淋巴结转移，影像学已经显示，就是时间问题。像这样有疫苗史，没有什么其他的，任何治疗（包括化疗或者是其他放疗）之前，尽快解刺突蛋白毒，同时青蒿衍生物疗程也可以帮助抗癌。

这个患者目前肝功能倒是正常的，他可以按照青蒿衍生物排毒疗程，多疗程治疗。但是辅助抗癌，我一直说要升级你的病毒预防方案，也就是维生素的高剂量——四五倍剂量、五六倍剂量。只要维生素C你能够耐受的话（胃肠耐受），更高剂量，每天8000毫克、1万毫克都可以。伊维菌素，建议她因为现在目前是远处器官转移，需要比较高剂量的伊维菌素，从低剂量开始尝试。因为很多人一上来高剂量受不了，很多人出现副反应，所以你可以从一毫克每公斤体重开始尝试；一周以后没什么不适反应，逐渐升高剂量，升到1.5毫克；再下一周没什么，再升到两毫克每公斤体重。两毫克每公斤体重是我推荐的转移癌患者的用药剂量。当然，欧美人体积比较大、脂肪量比较高的人，有的人用到2.5毫克，但是过高的剂量不宜，会得不偿失，因为你各种副反应出来，反而影响你的正常治疗。

伊维菌素一定要饭后用药，你可以分次用，哪怕三餐的饭后用都可以，饭后用，每周用5天。青蒿琥酯或者是其他青蒿衍生物（比方说双氢青蒿素之类的都可以），早餐前空腹用药。我说早餐前空腹用药有原因的，也是从低剂量开始，逐渐增加到患者的治疗量。每个人的治疗量不一样，根据个人的耐受性，像颅内肿瘤剂量可以很高，只要你耐受。通常的普通成人的不管解毒、抗癌剂量，200到300毫克青蒿琥酯的话，有些癌症患者因为血清游离铁水平很高，暂时不需要补充额外的血红素铁。但是几个疗程下来，你复查肝功能的时候，顺带便查一下血清铁蛋白来决定，你是否要补充血红素铁。疗程当中，就根据你这些复查结果来进一步调整你的用药剂量，不管是血红素铁也好，还是青蒿衍生物剂量也好，来帮助调整剂量。青蒿素类需要跟维生素/营养素分时三小时以上。你要是补充铁的话，要跟你每天用的锌分开来服用，一早一晚。所有方案都在我们节目末尾的汇集方案当中。
