Classic 3 Ps:-

Parathyroid adenoma causes Hypercalcemia, kidney stones.
Pancreatic neuroendocrine tumors causes Gastrinoma, Zollinger–Ellison syndrome.
Pituitary adenoma causes Hormone excess.

Gene:- MEN1 (menin), chromosome 11q13.

🔴 Trousseau sign- It is elicited by inflating a blood pressure cuff on the arm above systolic pressure for 2-3 minutes, causing ischemia.

• In hypocalcemia, this leads to carpopedal spasm- flexion of the wrist & metacarpophalangeal joints with extension of the interphalangeal joints, resembling the "obstetrician's hand."

✓ It indicates increased neuromuscular excitability due to low serum calcium levels.
• The sign is more sensitive & specific for hypocalcemia than Chvostek’s sign.

∆ Hypocalcemia can be caused by- Hypoparathyroidism, vitamin D deficiency, renal failure & other metabolic disorders.

This is classic for acquired methemoglobinemia induced by local anesthetics e.g., prilocaine, benzocaine, characterized by a "saturation gap" where cyanosis occurs alongside a falsely reassuring pulse oximetry reading.

Diagnosis: It is confirmed via co-oximetry showing elevated methemoglobin levels.

Mx- The definitive first line antidote is intravenous methylene blue.
