There are three major late-breaking trials presented this year at #EuroPCR (May 15, 8.30-10.00 AM CEST).
The first trial is called REC-CAGEFREE II. It is the first randomized study of antiplatelet monotherapy after PCI with drug-coated balloons. It will test a stepwise regimen consisting of aspirin plus ticagrelor for one month, followed by five months of ticagrelor monotherapy, and then aspirin monotherapy for six months in patients with ACS. The primary endpoint is a composite of ischemic and bleeding events at 12 months.
The second trial is called NOTION-2. It is a(nother) study comparing TAVI versus surgical aortic valve replacement (both arms will use any CE-marketed valve), but this time the study ispossiblymore controversial as the study enrolled younger low-risk patients than usual (75 years or less). The primary endpoint is a composite of all-cause mortality, stroke or rehospitalization. The reported follow-up will be short (12 months), so whatever happens... let's stay calm.
The third trial is called LANDMARK. It is a non-inferiority study of the balloon expandable Myval prosthesis (available with intermediate sizes) compared to other two contemporary transcatheter heart valves (Sapien and Evolut). The primary endpoint is a VARC-3 composite of safety and efficacy outcomes at 30 days.
For each trial, after the presentation, there will be a methodological appraisal (I will be handling NOTION-2 and LANDMARK) and a discussion on the possible impact of the study on clinical practice.
