It's not just about debt though, it's about employability to pay off that debt. A US Citizen cannot work as a physician in their home country unless they do their residency here.

@mahad_minhas I am not 100% sold on these tiers (wonder about some of tier 2 being absorbed into tier 1) but I appreciate your thinking and don't think it's xenophobic. It's possible to support immigration and FMGs and also think prioritizing US citizen (AMG and IMG) is needed in our system.

@EvilMoralless @londyloo @TheNRMP @ERASinfo @AAMCtoday I agree that part of the solution to the match is increasing spots and part of it is looking at the limits we have (or don't have) on visas to ensure a balance of AMGs to FMGs that is reasonable. Based on current numbers I am simply unable to attribute the entire problem to FMGs.

Immigrating foreign docs have agency, are not being forced to move. They are making a choice. Just like US docs who move to New Zealand do, even though we have a shortage here.
