Another uncomfortable truth for U.S. physicians.

American doctors aren’t “losing” because they lack skill. They’re losing because they start $250k–$500k+ in debt.

U.S.-trained physicians graduate owing that, then grind 3–7 years in residency on limited pay while interest piles up. Debt dictates every move: location, salary, risk-taking.

Foreign-trained often graduate with little/no debt from subsidized systems abroad. Many enter paid practice sooner via alternative state paths (18+ states) that skip full U.S. residency.

One repays massive debt from day one. The other earns and builds equity years earlier.

This isn’t merit.

When equally qualified doctors compete, but one services huge debt while the other starts building sooner, outcomes diverge predictably.

We call it a “shortage.” It’s policy-created asymmetry: restrict residency slots for Americans, then fast-track foreign doctors to fill the gaps.
