There were two cases with tokens that I know of. One was using 15 tokens facedown while playing domain monarchs (known for not having an extra deck). The other one had custom swordsoul tokens while playing another archtype, and admitted on a deck profile that the sole intent was to trick the opponent. Both were misrepresenting the gamestate.
