Then the consultant looks at you:
“So, what’s your diagnosis?”

The bulging fissure sign refers to exudative lobar consolidation that rapidly expands the affected lobe, causing displacement of the adjacent fissure. It has been classically described in right upper lobe consolidation secondary to Klebsiella pneumoniae
