IF you are asked to sign a vaccine refusal form, you DO NOT have to. A good physician will not expect you to sign one and will just document that they "counseled" you on vaccines and you declined them.
If you're in doubt of signing ANYTHING- Consult an attorney, I did consult ours that informed me:
