@CanadianKayMD The in-patient gridlock is a major issue, this is where discharge planning starts on admission in order to loosen the gridlock. Unfortunately there are too many clients and too few beds in residential/extended care.
@raghu_venugopal Part of the problem in BC was that beds were over utilized and when a review happened the number of available beds were cut severely. My local hospital went from 45 beds to 25 beds then to 12 and now at 8. We should have stayed at 12 to 15. π
@CanadianKayMD Wow, that sucks. In BC we had a discharge planning team who would cover the ER for difficult discharges. As a PT Iβd try to cover off the physical needs but OT and social worker were key players. Without a GP to rely on, the discharges are likely not going to work βΉοΈ
