@ashishkjha I've had chief medical officers from hospitals mention repeatedly the critical importance of being able to speak perfectly fluent English for their older patient base, and the lion's share of healthcare is delivered to an older patient base.

I'd assume that these patients are screened for symptoms first. That means the likelihood that they have the disease is no longer 1 in 1000 - it will be less than that. There is no way to come up with a correct answer.
