SEN. BORRIS L. MILES LETTER TO THE GOVERNOR
I sent a letter to Governor Abbott in response to his letter regarding the federal government's decision to withhold approval of Texas' FY 2027 Medicaid directed payment programs, which include the Comprehensive Hospital Increase Reimbursement Program (CHIRP), Texas Incentives for Physicians and Professional Services (TIPPS), and Rural Access to Primary and Preventive Services Program (RAPPS).
Approximately $9.8 billion in Medicaid funding for Texas hospitals is at risk, including at least $258 million for Harris Health.
Those numbers represent far more than hospital finances. The Texans most vulnerable to the consequences are our seniors, low-income families, minority and marginalized communities, uninsured and underinsured Texans, and residents of rural communities where healthcare options are already limited.
In the governor's August 7 letter, he described the federal government's action as putting a “gun to the head” of Texas hospitals. But if we are going to use that analogy, then we also need to be honest about how Texas arrived at a place where that gun can be held to the head of our healthcare system with such devastating consequences.
For years, Texas made a deliberate political choice not to expand Medicaid under the Affordable Care Act. Time and again, Texas policymakers had opportunities to expand access to Medicaid and chose not to do so. This refusal helped create and perpetuate a healthcare system in which hospitals—particularly safety-net hospitals—carry an enormous burden of caring for uninsured and underinsured Texans.
We cannot continue making the same policy choices that continue to leave us vulnerable. We must: Put people before politics. Put patients before ideology. Put human life before partisan advantage. The health and lives of Texans should never be collateral damage in a political dispute.
