NICE Guidelines for Primary Hyperparathyroidism NG132 https://t.co/3mYbYaEtRh continue to bamboozle the world of endocrinology. We hear endocrinologists tell patients, 'oh no, I can't refer you to surgeons unless you have kidney stones', and 'we won't operate until you have end organ damage.'

For crying out loud, the suffering we've seen since publication of NG132 is deplorable. @NICEGetInvolved I would urge you to share this with NICE Board members. Patients have endured seven years of doctors who can't get past recommendation 1.3.1. Even your committee, despite lack of evidence (we can provide so much evidence) agreed that surgery is beneficial and cost effective because it can be expected to result in a cure, and eliminate the need for further treatment. Yet this message has escaped @RCGP & @Soc_Endo despite being available to read (see below) in the 'overview' box/rational and impact: Referral for surgery
Recommendations 1.3.1 and 1.3.2
Why the committee made the recommendations.
I would advise you that ongoing/current practice of forcing hundreds of patients to wait for end organ damage goes against your cost effective recommendations, and is causing unnecessary harm, intolerable suffering and costing the NHS colossal financial expenditure. I can advise a very simple solution:

Delete recommendation 1.3.1 and put 1.3.2 in its place, urgently.

Hyperparathyroid UK members wrote to NICE in March 2021 and NICE board members in January 2024 (over 160 letters). They were dismissed because Parathyroid UK said your guideline is very good. Their focus is hypOparathyroidism. Their treatment recommendations for PHPT on the NHS website clearly reveal the extent of their understanding of HypERpara. Our focus has always been hypERparathyroidism, since 2/9/2014. https://t.co/3gumB0xe2n
If you do nothing else, please just remove recommendation 1.3.1.
