Tue 3/17 to Fri 3/20: 10 am to 2 pm - limited to urgent appointments and picking up medical devices (glasses and contacts)

Mon 3/23 to Mon 4/6 - on call, limited to urgent appointments only.  Staff will be available via phone or email.

Starting March 19th, Crest will no longer be in-network for Davis Vision Insurance. Crest continues to be in-network providers for: VSP, Eyemed, Superior Vision, MES, and Spectera. We are also in-network for many PPO medical insurance plans. Call us if you have any questions.
