@mancunianmedic@sophia_edw28123 Excellent summary David. Another risk in this two tiers is the lack of integration (notes & electronic records) & no published outcomes etc. Licensing should be stricter clearly as we’re now surrounded by range of new private providers. 🤷♂️ re quality
I’ve been doing our triage & advice/guidance service in renal medicine for 10 & 6 years respectively. Latter advice system well received & most docs (& patients) think it helpful adjunct to patient care.
However it has to have defined boundaries/guideline derived criteria, courteous exchanges & critically no or minimal transfer of new work to the community. Consultant has to thus spend more time than they think & always leave access open/option for consultant to phone patient (or GP) as reading between the lines is the new skill set. Elderly patients particularly like not coming to clinic for an eGFR question when asymptomatic. We are lucky having NICE guidance that covers a lot of the referrals/guidance & anecdotally we think we’re getting more patients on EBM meds earlier with no additional bloods, X-rays or visits. #WinWin
Belated happy Mother’s Day to all mums. Over 35 years since I said similar to my own mother. My passion for medicine comes from watching her suffer & die aged 62 from a myocardial infarction. Glad to say it wouldn’t happen now. Been a privilege to see that progress.

This is the little boy you all forget is at the centre of this...an innocent, beautiful child who had plans to study medicine and was excited for life. He was loved and adored, so think twice before you sully his memory anymore....

You will never cover medicine in didactic lectures- competent become capable learners meaning they can apply knowledge from first principles & have ongoing informal adult learning
