في هذا الكتاب حديث عام حول كفاءة الطب الذي يتم تجاوزه إلى من يمارسه على أرض الواقع!
فبينما ينجح الطب الحديث في نسخته التقنية من تحويل الجسد الإنساني إلى معادلة قابلة للتحليل، لا يحقق ذات النجاح في احتواء تجربة الطبيب الإنسانية وهو يمارسه، وهي المعضلة التي تكشف عنها المؤلفة كمنطقة مظلمة تتقاطع فيها المعرفة مع القلق، ويرافق الإنجاز الشعور بالذنب.
فالطبيب الذي يرزح تحت ضغط مستمر بين متطلبات العلم واتخاذ القرارات وسقف التوقعات الاجتماعية، يواجه محنة التصرف بالعقلانية المتوقعة التي غالباً ما يرافقها خطر فقد البوصلة الإنسانية التي تمنح قراراته معناها!
تنطلق المؤلفة في كتابها من تصوّر غير معلن يعكس صورة الطبيب المثالي كوهم ثقافي أكثر منها و��قعاً مهنياً ملموساً! فالطبيب كما تقدّمه التجربة المهنية، ليس كياناً محايداً مجرداً، بل كيان إنساني يعيش تحت ضغط مستمر يتز��حم بين المستجدات العلمية واتخاذ القرارات الصائبة والاعتبارات الاجتماعية.
وحيث التكوين الطبي نفسه يساهم في خلق هذا التوتر كما تعتقد المؤلفة، فإن الأطباء يُدربّون ضمنياً على إخفاء مشاعرهم التي تبدو كعائق أمام القرار الحكيم، في حين أنها في الحقيقة جزء لا يتجزأ من هذا القرار!

تقول المؤلفة في مقدمة كتابها:
"وبحلول الوقت الحاضر، يدرك حتى أكثر الأطباء تقليدية أن العواطف حاضرة في الطب على كل المستويات، لكنها غالباً ما تُدمج ضمن مفهوم عام يشمل التوتر والإرهاق، مع افتراض ضمني أن الانضباط الذاتي كفيل بضبطها. غير أن الطبقات العاطفية في الطب أكثر تعقيداً وانتشارا�� مما نتصور. ففي الواقع، كثيراً ما تكون هي العامل المهيمن في اتخاذ القرارات الطبية، حيث تطغى بسهولة على الطب المبني على الأدلة والخوارزميات السريرية ومعايير الجودة، وحتى الخبر�� الطبية.. وغالباً دون وعي من أحد! قد يُقال بسهولة إن الأطباء لا يختلفون عاطفياً عن المحاسبين أو السباكين أو فنّي إصلاح الكابلات، لكن محصلة سلوك الأطباء سواء كانت منطقية أو عاطفية أو غير عقلانية تحمل عواقب حياة أو موت بالنسبة للمرضى، أي بالنسبة لنا جميعاً".
