@TDekkersPhysio@marklaslett_NZ Would you explain this to the patient? It’s almost impossible avoid loaded flexion in life, putting the kids toys away or gentle gardening? I would also argue I see way more patients with fear avoidance and weakness driving their pain, and chronicity IMO

@elonmusk The deflection is crazy, you literally endorsed a convicted a lying rapist and con artist, you have zero experience of life in the UK and surely should be more concerned with the multitude of issues in the US?
@GregLehman I’d use them to measure variability in R/L limbs, and then only with patients who’s history implies pain started with an increase in activity their body couldn’t sustain, rather than actual diagnosis. Dyno would then be used to track progress.
@drshiller613@DerekGriffin86 Of course it’s complex, but exercise is one component of a good management strategy. Hard to make complex pronouncements in such a limited word count.
@DerekGriffin86@novicephysio Which terms and routes do you feel would be better, I feel like we use vocab that means nothing to patients for a while, until they become pathological, and then we move to new terms, and around it goes. Obese and chronic are good examples, harmful terms now 🤷🏼♂
@DerekGriffin86@novicephysio In basic ways that mean something to the patient as much as possible, reps/sets/time/distance of tasks and movements that are relevant. The main challenge is not using pain, not to begin with anyway
@DerekGriffin86@novicephysio Couldn’t be anything more complicated, I’m simply saying you can understand why encouraging robustness is done in clinical settings and that in lots of cases it’s appropriate, I think any nocebic impacts can surely be forgiven if it’s true… no?
@novicephysio@DerekGriffin86 Yes I’d agree the short term analgesic effects are more central in their impact, I spend much more time helping patients understand and be motivated by the long term effects though, which are broad and include load tolerance in lots of cases
@DerekGriffin86@novicephysio As much as I agree, and would love to believe the average patient could be motivated by that explanation, in the 20 minutes I get with patients to diagnose and explain things, a simpler justification of exercise is needed
@novicephysio@DerekGriffin86 It can, doesn’t always, chronicity of pain negates its effects, that’s just personal view not sure if there’s evidence of that per se. Can improve function also so, same pain but now able to do something that was too painful before is nearly as good as less pain
@DerekGriffin86 The danger is being too simplistic, numerous things affect pain and lack of physical fitness relative to an individuals needs is likely one. Being inactive is highly correlated with chronic pain. Like all things health, complex to fix once you’ve started the slide
@DerekGriffin86 Exercise has repeatedly been shown to be an excellent remedy for a lot of peoples back pain. Lots more to it for a lot of people but surely the implication is that some backs could do with a bit more 💪
