@TopQuokka@cjsnowdon Those are the words used in the root cause analysis for the triplet. I presume it means that the paediatricians failed to document their actions during the delivery of the twins. One has to wonder why. The parents both recorded blood spattering all over the walls.

@mellycatslave@Sunraged@ContrarianJolly All the experts kept on revising their opinions and especially Dr Evans. He wrote over 80 reports, sometimes as many as seven or eight for a single baby. Most of those reports were written after Lucy Letby had been arrested.
