@macaron_md_ You’re completely right. However, we never intended to transfer our conclusions into daily practice-the main aim of the course was to give the participants the opportunity to test their limits in a controlled environment under dedicated guidance and support.
We need to talk about this
☠️Azithromycin and Amoxyclav for every run-of-the-mill viral sore throat
☠️Cefuroxime for every viral cold and cough
☠️Upgrading to Meropenam from Ceftrixone when a single fever spike persists in the wards
☠️Upgrading to Colistin with a single fever spike on Meropenam in the ICU
☠️Starting Ceftazidime and avibactam when patient enters the ICU
☠️Not stopping previous antibiotics but adding on more antibiotics in the ward and ICUs like a domino effect
☠️Not de-esclating or stopping antibiotics on time when body fluid cultures come back sterile
☠️Over-the-counter antibiotic sales without prescriptions
💀Alternative medicine practitioners prescribing antibiotics and antifungals without a clue, when Ayurveda or Homeopathy never had a germ theory
☠️People taking antibiotics unnecessarily and for the wrong indications AND stopping after 2 or 3 pills, that too taken erratically
☠️Irrational and illogical antibiotic use domesticated/ farmed-for-food animals and in veterinary practice
☠️Medical representatives asking doctors to prescribe a new antibiotic because it is better than the rest and more "powerful" without knowing recommendations for use - and ill-informed doctors learning medicine from medical representatives on antibiotics use.
🚨This depressing list is endless🚨
This is a complete human-made disaster that needs reflection from every treating doctor to follow what is necessary to reverse the damage done and to prevent irreversible damage in patients who will one day require life-saving antibiotics.
Patients with liver disease and cancer will be the worst hit because of this foolish and irrational antibiotic use. Brace yourselves.
I’m always astonished that these “my daily routine” posts (even the more sincere ones) never include any type of household work. Cleaning, doing dishes, tidying up, cooking, etc.
And my daily routine:
I wake up at 5 am.
Get to the gym at 5:30 am.
While working out, I listen to a science podcast for 1h.
Have breakfast and respond to emails at 7:30 am.
Reach to my lab at 8 am.
Work until 6 pm.
Have a dinner at 7 pm.
Read research reviews for 60 mins.
Read a book for another 60 mins.
Write down the work plan for the next day.
9 pm: I’m in bed, thrilled to start another super-day in 8 hours.

Sounds perfect, right?
Except that this is not real. It’s NOT about me.

In real life:
- I can get sick of office work
- I can get sick of writing grants, papers, reports
- I can get sick of working in a lab (hell, yes!)
- I can get too tired for a gym

My life as a scientist is not about productivity.
It’s about creativity and fulfillment.
Such a daily routine can quickly exhaust us, mentally and physically.

As a student, postdoc or faculty:
Do NOT feel guilty when you get sick of your lab duties.
Do NOT feel ashamed of your slow writing process.
Do NOT get upset by your mental and physical fatigue.
▫️
Your strength is in your personality and curiosity. Not a productive daily routine.

Ach interessant, das letzte Mal wurde die Sau vor fünf Monaten durch #medtwitter getrieben. Ernährung ist eine große Stellschraube, die deutsche Krankenhausleitungen glauben nicht zu brauchen, weil man ja bessere/mehr Medikamente und moderneres Material im OP hat (?)
An dieser Stelle muss ich sagen, dass es in dem (staatlichen) indischen Krankenhaus, in dem ich war, jeden Tag Linsen, Reis und Gemüse gab mit entsprechenden Anpassungen für bestimmte Patienten. Dort waren die Ärzte total schockiert, als ich ihnen von deutschem Krankenhausessen
@rosarote_viggo Von der Referenzgruppe (also Erstteilnahmen mit Mindeststudienzeit) allerdings nur 6,5%, was konsistent mit den Zahlen der Jahre zuvor ist (ca. 5% durchschnittlich). Im Herbst sind die Durchfallquoten fast immer niedriger, weil die Referenzgruppe >80% der Teilnehmenden ausmacht.
