A journey of a thousand miles begins with a single step. Public health has quietly contained these for decades. Only our failures made the press. Public Health work is confidential except for a need to know. So if a family is quarantined it is not published. And the outbreak stops. But if that family is not contained and it spreads then soon there are many families that need to be contained. So the children who died between 90 and 120 who did not receive any vaccine are not in the study because they could not have gotten a vaccine after they died.

There is no question that public health measures including clean water, sanitation, treatment of infectious diseases, quarantine and isolation. But vaccines have also decreased infectious disease mortality.

The case review does not provide enough information and even if it did I do not have the skills or knowlege to make a judgement. I do not know if there was an autopsy. Hopefully the hospital has a quality improvement committee that can look at all the information and determine where improvements can be made and make the improvements. No human, or system is perfect and continuous quality improvement keeps us all safer. Decades ago when I trained in pediatrics and infectious disease Mycoplasma pneumonia was literally called walking pneumonia. Why? Because we humans would often get it, still be able to function and usually recover even without antibiotics. For a child to be on a ventilator from it would have been highly unusual BUT measles can depress the immune system so anything is possible including more than one pathogen. And I am not currently up on any mutations etc so the bacteria may have changed.

How can you know the "right" antibiotic before the culture results and their antibiotic sensitivities come back? You can't. So when pneumonia is first diagnosed (by xray and or physical exam) the clinician choses the most likely bacteria and/or virus but NO ONE can know for certainty until the culture results come back. And even then if someone is severely immunocompromised from measles even antibiotics/antivirals the bacteria and or virus are sensitive to may not work.
