Did you know that if you live in the US you do not need a prescription to receive physical therapy services for your Parkinson's disease? Did you know your PT can work with you to choose the type and frequency of PT that fits your needs? Check out this fresh new survey. The Physical Therapist can also help to educate, recommend and in some cases monitor the appropriate roll out of an exercise program. PT for neurological disorders is most commonly used as a ‘burst’ of short-term, closely spaced visits over 6–12 weeks, however the field is shifting to appreciate the importance of maintaining PT in PD over time. This can be accomplished w/ regular PT assessments to determine optimal regimens.
This recently published survey by LeLaurin and Salloum @FixelInstitute@UFHealth focused mostly on 'neurologists' though the information is still insightful. 'Over 90% of respondents reported that folks at their sites participated in burst PT while about half of respondents reported patients participated in spaced PT. 'Two sessions per week was the most common timing of PT in about half of the sites, followed by one session per week among 26.8 % of sites.'
My take: We need to teach folks with PD that they can 'self-refer for PT.' Additionally, we need to honor PT's and empower them through placing a greater emphasis on the benefits of PT. We (neurologists, general doctors, advanced practice providers) can do a better job educating and a better job in making regular appropriate referrals. PT's have an expertise that is underutilized. The national data strongly suggest we are under-utilizing PT in PD. We should consider PT's as the experts who can implement published guidelines and can assess in real-time the type of of therapy, the duration of therapy and also can decide on the appropriate application of a maintenance program. Regular exercise and PT in Parkinson's is supported by guidelines, and in my view we need to continue to empower folks with PD to self refer, practitioners to refer and PT's to continue to implement the published guidelines.
