Puzzling case of this patient with collapsing FSGS since 2016, admitted with worsening LE edema. Historically all UAs with >500 mg protein but MOST UPCR and UACR are negative. 24 hour urine collection done yesterday with undetectable protein (adequate collection) 🤔🤔

Interesting case of spontaneous hypoPTH. Previously healthy 32 yo M presented with tingling, cramps, found to have Ca 5, undetectable PTH. normal kidney fn, mild rhabdo. Suspect autoimmune/ post viral etiology. Genetic testing pending..

Interesting case of hypoNa 2/2 Biliary losses. Pt admitted w/jaundice, found to have mass obstructing CBD. Developed ATN from dehydration, poor PO intake, contrast and ketorolac. Anuric, not hypervolemic, developed hyponatremia post-biliary drains w 2-3 L output. Chkd Na in bile:

Average Na concentration in bile is around 145 mmol/L (usually varies between 122 and 164 mmol/L). In our case, the biliary drain brought back a daily volume of about 2 liters each day, which represents about 258mmol/ equivalent to a loss of about 6 g of sodium per day.

@VelezNephHepato C3 and C4 are normal. urine spun and it was very bland. UPCR repeated - 4 gr first time and 6.5 second time. GN serology pending. I think I will go ahead and biopsy as there is so much uncertainty

44 yo w/ foot infection- failed rifmapin, levaquin, augmentin Tx, admitted 7 days ago with mrsa bacteremia. No h/o kidney disease. Required pressors briefly, on vanc/cefepime (vanc levels always <22) has progressive AKI non-oliguric. UPCR 6.5 g/g. 6-10 RBCs.

creatinine keeps going up (7.6 today), but BUN remains 43-47. Has asterixis, not on any Meds that block secretion of creatinine. Urinates 1.5-2L/day. Would you biopsy?
