My Lp(a) is 175. I have been waiting for this trial for years.

Yesterday, Novartis released the results of HORIZON. The biggest Lp(a) outcomes trial in history. 8,323 patients. A drug built from the ground up to do one thing: lower Lp(a).

The drug worked. The cardiovascular events did not change.

This is the most important failed trial in cardiovascular medicine this year. Nobody is explaining why it failed.

It is difficult but there are certain parameters like
1. If surface is irregular ( due to disrupted fibrous cap ) it goes in favour of eruptive CN where smooth convex surface goes in favour of non eruptive one .
2. Any visible thrombus within the CN goes in favour of eruptive CN .
Having said that OCT is always better .

@drAliyor @Sher_Intervent This is not calcified nodule . This is just an eccenctric calcium . A calcified nodule is a distinct calcium mass protruding into the coronary lumen with a convex, luminal surface. Here we see the surface is flat or slightly concave .

A middle-aged male presented with unstabe angina. He had a tight stenosis in mid RCA. The IVUS pullback after doing pre dilatation revealed this image which appeared like a Yin and Yang symbol.

Couple of observations :
1. artery is plaque free ( atherosclerotic cause ruled out )
2. The entire vessel size is reduced ( EL-EL is reduced ) . This happens in two conditions either it is spasm or there is some external compression of the ostium
3. Some thickening of the media is also seen

If it is after predilatation then it could be a dissected plaque with accumulation of blood within the medial space .

1. Govt issues rules to make flying safer in India.

5. Don't issue cancellation advisory in advance.

6. Make public suffer for 24hrs inside airport by saying flight delayed. Knowing massive public noise would be created.

7. Refund the money after 24 hrs.

8. Earn interest on the money for months since people booked it.

9. Let entire nation suffer, by choking aviation transport totally.

10. Govt rollback the rules.

CT Calcium Score of Zero- how useful is it?

In ~2000 patients with a zero CT calcium score,

0.5% had coronary artery disease seen on CTA which did not affect prognosis.

During a 13-year follow-up period, none died of a coronary event.
