<30% of the respondents got it right. Though it looks like a pulsatile PV at first glance, you must pay attention to 2 things here
- Scale inversion: Blue is on top of the color bar - meaning flow TOWRDS the transducer is BLUE as opposed to traditional BART (blue away-red towards). In spectral Doppler, negative scale is above-the-baseline and positive scale is below.
- Anatomy: the blue vessel is going into the liver from outside - portal vein.
Without EKG, it's difficult to identify the HV waveforms but I would 'guess' S-D fusion based on the symmetry.

Quick Case.

Patient with a history of recent trauma. He was referred for an ankle ultrasound (normal).

Clearly, the problem is at the mid-foot.

Lisfranc injury, maneuvers are performed with stress, axial compression on the metatarsal with a fixed midfoot, and we can clearly see the marked instability of the medial column, with opening and displacement.

It is much more evident when compared to the normal side.
