Left hip doesnt look dysplastic. I would use It as a reference for lenght and offset... but the actual femoral version and canal will be totally different in the affected side. So I wont choose the femoral stem by looking at the normal hip.

Case wrap up. 55 y fem, 1.48 mts
Dysplastic
Posterolateral approach
Trident PSL 50. Decides to elevate de CoR for better coverage and get 36 head. 1 screw "just for the picture". Femoral anteversion 35-40 degrees intraop. Cemented Exeter 35.5 offset, 32+4 ceramic head.

Fem, 55y. O/w healthy. 1.45 mts tall (4.8 ft). 5 years of severe R hip pain. Femur ext rotated so difficult to determine true offset and any metaph/diaph missmatch.

These are the preop templates of both sides. Left: Pinnacle 46 with Cstem 1 standard (37.5 offset). Right 48 cup. Might use Triden PSL for 32 head if 46/ Exeter 37.5

There is growing evidence in favor of Replacement but in elderly (>75), comminuted, complete or posterior tilted (>20) garden "1-2".

Plan:
Supine, traction table, ext rot to match cortical readings, clamp, gradually int rot to bring tip of GT to position the starting pin.
