Over 20 years ago, I watched 45 minutes of resuscitation end in silence. The weeks before were filled with subtle symptoms, easy to dismiss. Until they weren’t.
That experience never left. It provided #lessons
• Not all risk announces itself
• Delayed recognition is common
• #Medicine has limits
• Sitting with patients in uncertainty is part of the work
• Community outreach can increase awareness and change outcomes

AV Nodal Reentrant Tachycardia (AVNRT)
A common type of supraventricular tachycardia (SVT) seen in young adults and structurally normal hearts.
Basic Concept:
AVNRT happens because the AV node has two pathways that conduct electrical impulses differently:
1️⃣ Slow Pathway
-Conducts impulses slowly
-Recovers quickly (short refractory period)
2️⃣ Fast Pathway
- Conducts impulses quickly
- Recovers slowly (long refractory period)
Mechanism of How AVNRT Starts:
🔻Normally, impulses travel down the fast pathway to the ventricles.
🔻But if a premature atrial beat (PAC) occurs while the fast pathway is still refractory (not ready), the impulse gets blocked there.
🔻Instead, the impulse travels down the slow pathway.
🔻By the time the impulse reaches the bottom of the circuit, the fast pathway has recovered, allowing the impulse to travel back up retrogradely.
🔻This sets up a circular electrical loop (reentry circuit), creating AVNRT.
This results in:
✅ Forward conduction to the ventricles via the slow pathway
✅ Backward conduction to the atria via the fast pathway
ECG Features of AVNRT:
🔹Regular rhythm
🔹Narrow QRS complexes
🔹No visible P waves or pseudo R’ in V1 or pseudo S waves in inferior leads (due to near-simultaneous atrial and ventricular activation)
🔹Heart rate: usually 150–250 bpm
Clinical Significance:
🔸Presents as palpitations, lightheadedness, dizziness, or even syncope
🔹Often triggered by stress, caffeine, alcohol, or exercise
🔸Not usually life-threatening but can be very symptomatic
🔹Responds well to vagal maneuvers or adenosine
🔸In recurrent cases, radiofrequency ablation of the slow pathway offers a cure
