As a haematologist who treats blood clots daily, it's reassuring to see even the resources of a $10 billion franchise can't always call it perfectly.
Like football, thrombosis care is a game of inches; balance risk vs benefit. So many variables to account for in each side.
The NFL and the Philadelphia Eagles almost killed Jalen Hurts for a Week 5 international showcase.
The full details of what happened behind closed doors in London just surfaced, and the negligence is staggering.
Before kickoff in London, Hurts was in a hospital bed until 4:00 AM undergoing emergency vascular testing. Medical staff instructed him not to sleep because sleep was considered an active health risk.
Instead of shutting down their $255 million franchise quarterback, the team cleared him to play 60 minutes of brutal, high-impact collision football on zero sleep.
Then, they loaded him onto a pressurized, 8-hour transatlantic flight back to Philadelphia.
Every doctor on the planet knows that long-haul flights and cabin pressure are the exact triggers that cause lower-extremity blood clots to break free. The Eagles spent days assuring the public it was just a "minor, superficial calf clot".
Now the truth comes out: the clot dislodged, shot through his bloodstream, and caused multiple pulmonary emboli in his lungs.
Hurts is officially out for the season, placed on blood thinners where taking a single routine tackle could cause a fatal internal hemorrhage.
He isn't just lucky to play football again, he is lucky to be alive.
When the NFL preaches "player safety," remember this moment. They ran a human being into the ground for television ratings in the UK, almost lost him to a preventable medical catastrophe, and cratered their entire season in the process.
Who inside that building signed off on letting him play and board that flight?
Haem/Once clinic communication 101 for new diagnoses:
- Less is more! Be concise, especially if you tend to talk more
- Talk slower than feels natural
- Check understaning, summarise, use big picture/bottom line statements as much as possible
3 mistakes people make when trying to learn a new skill.
- Overcomplicating your plan instead of building a simple daily habit.
- Overestimating your ability+time instead of starting small + consistent.
- Underutilizing spare time eg the commute!
3 mistakes trainees make when prepping for consultant life:
- Assuming training board automatically notifies GMC (they don't!)
- Thinking opportunities stay the same as Cons (some are just trainees only!)
- Being indecisive about jobs - start early, think and planbyour ideal
During FY/intern year I was told "you're too nice to work here" (elderly patient on trauma unit) + "you should learn to say no" (my consultant during medical training)
As a type green/people pleaser, I'm still working on these but this advice is spot on. Work on yourself first.
My advice for the newish interns.
Take responsibility.
The right mentality, nothing will happen unless I do it. Check everything with the source, patient and raw data (labs, imaging reports etc).
Do not trust anyone (sometimes even your attending) and independently verify.
AI will outperform doctors soon, if not already.
3 mistakes NHS staff make when thinking about AI:
1. Panic - jobs, risk, pt safety!
2. Not using AI already to help workload
3. Not learning digital healthcare principles (read @Bob_Wachter)
Think big, learn now, do well

- protect a sacred hour
- get up early
- think or write on their commute
- avoid procrastination
- bias to action
and don't overthink

@Nigel_Farage Just got in from a busy A&E shift with staff from EU countries working their ass off to help U.K. citizens to see this comment Nigel. When you come in need of the NHS’s help you will come to rue the absence of these people your lies and poison have driven from our country
