CA data: 10 hospices at one Van Nuys address (shared with 204 others) filed cloned 2024 reports: 5 patients, 698 care days, 1 death, $1 Medicare revenue. They slightly altered visit counts (to evade automated detection?). This is a red flag. Did @Cal_HHS or @DHCS_CA investigate?
CA 2023 & 2024 data: 67 hospices reported < 0.05 nursing visits per day (median was 0.24) AND less than a 25% death discharge rate (expected is 75%-80%). These aren't clinical outliers, they are bare-minimum care facilities running a live-discharge model. Where is @Cal_HHS and @DHCS_CA?
Of those 67 hospices, 43 reported revenues of $88M during only those years in which they triggered at least a DHCS examination: near-zero nursing visits combined with impossibly low death rates. $88M paid for seemingly little nursing care for apparently non-terminal patients.
Hospice requires a terminal prognosis. Yet 30 CA hospices operated for 7 straight years through 2024 with death rates below 50% every single year. The clinical norm is 75-80%. These hospices received $359M in payouts. Has @Cal_HHS investigated these 30 facilities?
This is not a statistical anomaly. This is a pattern of sustained, systematic inappropriate enrollment that persisted without interruption from 2018 through 2024 β through the COVID pandemic, through multiple regulatory cycles, through seven annual report submissions to HCAI.
A terminal prognosis is required for hospice. Yet in 2024, 165 CA hospices reported a death rate of <1% (norm is 75-80%), meaning nearly 100% live discharges. They walked away with $177M in total payments. Why and how does @Cal_HHS pay out $177M to hospices where nobody dies?
Another red flag in 2024 CA hospice data: 300 of 1,767 active hospices pulled in over $800 in Revenue per Visit (median was $480). Medicare pays a flat daily rate, regardless of the volume of services provided that day. Are hospices collecting checks but skipping physical visits?
@Cal_HHS DOJ arrested an Artesia hospice owner for $9M in fraud, citing an 85% live discharge rate (5X the national average). Yet 2024 data shows 677 other CA hospices also hit that 85% rate. Why does it take the DOJ to catch what @Cal_HHS is completely ignoring?

A forensic review into 2024 Californiaβs 1,767 active hospices reveals a massive anomaly: the median death rate is just 40%. Standard end-of-life care averages 70-95%. Half the hospices discharged β₯60% of patients alive. Has HHS investigate this indicator of Medicare fraud?
βActiveβ hospices are only those facilities that recorded patients, revenues, and visits in 2024. Other hospice-only facilities may have operated, but they did not report one or more of these three items.
The extremely low death rate could indicate widespread enrollment of patients who are not dying. Are hospices systematically enrolling non-terminal patients to harvest Medicare per-diem payments, only to discharge them alive before exceeding federal annual billing caps?
@Cal_HHS How do you provide hospice care without seeing the patient? In 2024, 171 of CAβs 1,767 active hospices averaged under 20 staff visits per patient (median was 40). Billing taxpayers for daily enrollment while limiting patients of staff support hopefully triggered @Cal_HHS reviews.

@GovPressOffice After Newsom looses in the primaries, there should be a number of places that could be a great match for @GovPressOffice staff, given their tremendous skills and experience.
