CA data: 10 hospices at one Van Nuys address (shared with 204 others) filed cloned 2024 reports: 5 patients, 698 care days, 1 death, $1 Medicare revenue. They slightly altered visit counts (to evade automated detection?). This is a red flag. Did @Cal_HHS or @DHCS_CA investigate?

CA 2023 & 2024 data: 67 hospices reported < 0.05 nursing visits per day (median was 0.24) AND less than a 25% death discharge rate (expected is 75%-80%). These aren't clinical outliers, they are bare-minimum care facilities running a live-discharge model. Where is @Cal_HHS and @DHCS_CA?

Of those 67 hospices, 43 reported revenues of $88M during only those years in which they triggered at least a DHCS examination: near-zero nursing visits combined with impossibly low death rates. $88M paid for seemingly little nursing care for apparently non-terminal patients.

Hospice requires a terminal prognosis. Yet 30 CA hospices operated for 7 straight years through 2024 with death rates below 50% every single year. The clinical norm is 75-80%. These hospices received $359M in payouts. Has @Cal_HHS investigated these 30 facilities?
This is not a statistical anomaly. This is a pattern of sustained, systematic inappropriate enrollment that persisted without interruption from 2018 through 2024 — through the COVID pandemic, through multiple regulatory cycles, through seven annual report submissions to HCAI.

Another red flag in 2024 CA hospice data: 300 of 1,767 active hospices pulled in over $800 in Revenue per Visit (median was $480). Medicare pays a flat daily rate, regardless of the volume of services provided that day. Are hospices collecting checks but skipping physical visits?
