Unedited tough POEM using Bipolar speedknife during Saudi Live 2026. There is almost no submucosal space for 5 cm above GEJ. In this video, you can see that water irrigation, with minimal cautery and careful dissection near the muscle layer, resulted in successful tunnel creation in this challenging case. It was such a pleasure working with Dr. Sarah Alghamdi on this tough case.
