I agree that each indication needs its own clinical evidence and regulatory approval, and some programs will fail.
That is exactly the valuation risk : investors may already be paying for successes that remain unproven.
Basically, exceptional melanoma data would get the platform thesis through to the next round, not win the whole tournament
MODERNA — BEFORE THE READOUT October 24
$MRNA $MRK — We know the trial succeeded. How strong was the success?
On September 25, we examined whether Moderna was still worth buying after its cancer-vaccine rerating.
Our central point was simple:
The scientific case had improved dramatically. The investment case still depended on the price paid for that progress.
On October 24, at ESMO (European Society for Medical Oncology: the medical society hosting this major annual cancer congress), we reach the next checkpoint: the detailed Phase 3 INTerpath-001 results.
This presentation should help us judge the strength of the melanoma evidence beyond the announcement that the trial succeeded.
Here is our framework, defined before the data.
1/ THE SETUP
INTerpath-001 enrolled 1,137 patients whose high-risk stage IIB–IV melanoma had been completely removed surgically.
It compared intismeran autogene, Moderna’s individualized mRNA (messenger RNA: genetic instructions that cells use to produce proteins) cancer therapy, plus Merck’s Keytruda against placebo plus Keytruda.
In August, the companies announced statistically significant and clinically meaningful improvements in:
• RFS (recurrence-free survival: the time before the cancer returns or the patient dies).
• DMFS (distant metastasis-free survival: the time before cancer spreads to distant sites or the patient dies).
No new safety signal was reported. Follow-up continues for OS (overall survival: the time until death from any cause).
The main uncertainty has therefore changed.
We already have a positive Phase 3 announcement.
Now we need the magnitude, precision, durability and breadth of the benefit.
2/ WHAT IS THE BENCHMARK?
The smaller Phase 2b KEYNOTE-942 trial enrolled 157 patients with resected stage IIIB–IV melanoma.
At approximately five years, it reported:
• RFS HR of 0.51.
HR (hazard ratio: a measure comparing how frequently an event occurs over time between treatment groups; below 1 favors the treatment being tested).
• DMFS HR of 0.41.
• Five-year RFS: 68.8% versus 49.1%.
The OS estimate also favored the combination, but its wide confidence interval included no difference. An OS benefit has therefore not been established.
These are compelling reference points. They are also estimates from a small trial.
We should not require Phase 3 to reproduce an RFS HR of exactly 0.51.
And including earlier-stage patients does not automatically mean the relative benefit must weaken. A lower baseline recurrence risk can reduce the absolute benefit without necessarily changing the HR.
3/ THE SIX QUESTIONS
QUESTION 1 — How strong is the RFS result?
This is the first number I will look at.
Our provisional interpretation:
RFS HR ≤0.60: exceptional magnitude.
Above 0.60 to 0.65: very strong Phase 3 efficacy.
Above 0.65 to 0.72: potentially substantial benefit, with more weight placed on absolute outcomes and valuation.
Above 0.72: a more modest relative effect than our preferred scenario.
These are our analytical reference points. They are not regulatory cut-offs, a verified market consensus or predictions of the stock’s reaction.
There is no biological cliff between 0.65 and 0.66.
The confidence interval, number of events and follow-up matter alongside the headline HR.
For newcomers: an HR of 0.65 represents an estimated 35% reduction in the event hazard during follow-up. It does not mean that 35 additional patients out of every 100 avoid recurrence.
QUESTION 2 — Does DMFS reinforce the story?
Preventing distant metastatic recurrence is particularly consequential.
We want a convincing DMFS effect supported by its confidence interval, event count and absolute rates.
A DMFS HR at least as favorable as RFS would be encouraging. It is not a requirement: the endpoints include different events and may have different maturity.
They also overlap. They should not be treated as two independent replications.
Strong RFS plus strong DMFS would make the clinical package more persuasive.
It would still not, by itself, establish cure or longer OS.
QUESTION 3 — How large is the absolute benefit?
Investors will focus on the HR. Clinicians and patients will also ask:
How many additional patients remain recurrence-free at a defined time?
We need the RFS and DMFS rates in both groups at adequately supported landmarks — for example, 24 or 36 months if the follow-up allows.
The time horizon is essential.
A ten-percentage-point difference at three years and the same difference at five years describe different observations.
We should therefore avoid using the Phase 2b five-year gap as an automatic pass/fail threshold for an earlier Phase 3 analysis.
The curves matter too:
• When do they separate?
• Does the separation persist or widen?
• How many patients remain at risk later?
A dramatic-looking curve tail supported by very few patients deserves less weight.
QUESTION 4 — How broad is the benefit within melanoma?
Stage II deserves particular attention because it expands the population beyond KEYNOTE-942.
We want coherent evidence across stage II and stage III/IV, interpreted alongside baseline risk and absolute benefit.
We do not need every subgroup to achieve statistical significance individually.
A significant result in one subgroup and a nonsignificant result in another do not establish that the treatment works differently between them.
Confidence intervals and interaction analyses will help distinguish a credible difference from statistical noise.
Biological analyses could also provide useful context, including:
• PD-L1 (programmed death-ligand 1: a protein that can help suppress immune responses against cancer).
• Tumor mutational burden — a measure of how many mutations a tumor carries.
• ctDNA (circulating tumor DNA: fragments of tumor-derived genetic material found in the bloodstream).
These analyses may help explain who benefits and why, if presented.
Exploratory findings should remain exploratory.
QUESTION 5 — Can patients tolerate and actually receive the treatment?
The larger Phase 3 dataset should clarify:
• Grade 3 or higher treatment-related toxicity.
• Serious adverse events.
• Immune-related toxicity.
• Treatment discontinuations and completion.
This is adjuvant treatment — treatment given after surgery to reduce the risk of the cancer returning. Patients have no detectable disease, so added treatment burden matters.
Then comes the practical pathway:
Tumor sample → sequencing → neoantigen selection → individualized mRNA design → manufacturing → quality control → delivery.
Neoantigens are abnormal targets created by tumor mutations that the immune system may recognize.
We want to understand how many patients successfully move through each step, how long it takes and why some do not receive treatment.
A high manufacturing success rate after successful design is not the same as a high success rate for everyone entering the pathway.
The timing of those steps relative to randomization matters too.
If detailed manufacturing metrics are absent, that leaves an open commercial question. It does not demonstrate a manufacturing failure.
QUESTION 6 — What does this establish about the broader platform?
This brings us back to our September investment case.
An exceptional melanoma result would strengthen confidence in the approach and the rationale for testing it elsewhere.
But kidney, bladder and lung cancer still require their own clinical evidence.
Likewise, success after surgery cannot establish efficacy in metastatic disease — cancer that has spread to distant parts of the body — or activity without Keytruda.
October 24 should clarify the strength of Layer 1 in our framework: melanoma.
It cannot establish the later layers.
4/ OUR SCENARIOS FOR OCTOBER 24
Very bullish
RFS HR ≤0.60, convincing DMFS, durable absolute separation, coherent subgroup results and manageable treatment burden.
Our reading: an exceptionally strong melanoma package, increasing confidence in further development.
Bullish
RFS HR above 0.60 to 0.65, corroborating DMFS, meaningful absolute benefit and broadly consistent results.
Our reading: excellent Phase 3 evidence. Investment upside still depends on the expectations already embedded in the price.
BASE
RFS HR above 0.65 to 0.72, positive DMFS, with a moderate absolute benefit or unresolved questions about breadth and maturity.
Our reading: a successful treatment, with a continuing valuation debate.
DISAPPOINTMENT RISK
RFS HR above 0.72 and/or a less convincing overall package: small absolute gains, credible subgroup differences, or material tolerability and delivery issues.
Our reading: potentially difficult for elevated expectations, despite a positive trial.
These scenarios concern the complete dataset. A point estimate alone should not determine the verdict.
A clinically positive result can still disappoint investors. An excellent result can also be largely anticipated.
5/ WHAT ABOUT OVERALL SURVIVAL?
OS follow-up is ongoing.
A favorable estimate would add support. Immature or near-neutral OS would not, by itself, negate strong RFS and DMFS.
An unfavorable numerical imbalance would deserve investigation: how many deaths, from which causes, with what uncertainty and follow-up?
With few events, both favorable and unfavorable estimates can be unstable.
We should not demand mature OS from an interim analysis.
6/ THE VALUATION TEST
Our September post argued that Moderna’s rerating appeared to reflect expectations extending beyond melanoma.
That remains an investment interpretation. The share price does not reveal exactly how much value investors assign to each indication.
The detailed melanoma results can strengthen or weaken the assumptions supporting a valuation model.
They cannot produce a fair value on their own.
That still requires estimates for eligible patients, adoption, pricing, manufacturing costs, margins, launch timing and Moderna’s economic share — alongside its existing business, other assets and future spending.
A lower HR strengthens the clinical evidence. The investment return also depends on what we pay for it.
7/ THE CALENDAR
October 24 — 16:30 Paris/Madrid / 10:30 a.m.
New York: announced ESMO Presidential Symposium session for LBA1 (Late-Breaking Abstract 1: the congress identifier for this presentation of newly available clinical findings), presented by Georgina V. Long.
October 24 — 19:00 Paris/Madrid / 1 p.m. New York: Moderna investor webcast.
October 25: Phase 1 pancreatic cancer poster — early clinical evidence.
October 26: INTerpath-014 high-risk stage I NSCLC (non-small cell lung cancer: the most common major category of lung cancer) poster — trial design, not an efficacy readout.
WHAT WE WILL TAKE AWAY
In September, we laid out the investment thesis.
Before October 24, we are defining the evidence needed to reassess it.
The first number will be the RFS hazard ratio.
The judgment will depend on:
Magnitude + precision + absolute benefit + DMFS + follow-up + breadth + tolerability + delivery.
A great melanoma result would be a major achievement.
We will still have to answer the next question : how far that success can travel and how much of that journey investors are already paying for.
Science first. Investment second.
MODERNA — INVESTMENT CASE
$MRNA Is Moderna still worth buying after the cancer-vaccine rerating?
This is one of the most interesting biotech investment cases right now.
Because Moderna has become a much better company.
But that does not automatically mean the stock has become a better investment.
Before the latest cancer trial result, MRNA was around $63.
Today it is around $190.
What changed?
A lot.
But so did the price.
The big event was INTerpath-001.
This was a large Phase 3 trial testing intismeran autogene, Moderna’s personalized mRNA cancer therapy, together with Merck’s Keytruda in patients whose high-risk melanoma had been surgically removed.
In this case, the treatment is individually designed for each patient to help the immune system recognize proteins created by mutations in that person’s cancer.
The trial succeeded on its two main measures:
RFS — recurrence-free survival — meaning how long patients remained alive without their cancer returning.
DMFS — distant metastasis-free survival — meaning how long patients remained alive without the cancer spreading to distant organs.
Both were significantly better with intismeran + Keytruda than with Keytruda alone.
That matters.
For years, personalized cancer vaccines were an exciting idea without definitive late-stage proof.
Now we have a positive randomized Phase 3 trial.
The scientific risk has fallen dramatically.
But here is the investment problem:
MRNA almost tripled before investors even saw the detailed results.
And that exposes an important paradox in biotech investing.
Waiting for proof protects you from clinical failure.
But sometimes the proof itself destroys the cheap entry point.
So what are investors actually buying at around $190?
Probably not melanoma alone.
That is the key.
Moderna does not keep all the economics from intismeran.
Development costs and profits are generally shared 50/50 with Merck.
Even a very successful melanoma treatment would therefore struggle, by itself, to explain the roughly $50B+ of market value Moderna gained after the announcement.
The market is already pricing something bigger:
The possibility that the same approach works across several different cancers.
And that is where the real bull case begins.
Moderna and Merck now have nine Phase 2 or Phase 3 trials studying intismeran in cancers including melanoma, lung cancer, kidney cancer and bladder cancer.
Think of the evidence ladder like this:
1 /Melanoma positive
→ intismeran is probably a real cancer drug.
Melanoma + kidney or bladder cancer positive
→ first strong evidence that the concept works outside melanoma.
2/ Melanoma + kidney + bladder positive
→ increasingly difficult to dismiss this as something specific to melanoma biology.
3/Then comes lung cancer.
NSCLC means non-small cell lung cancer, the most common major category of lung cancer.
If a Phase 3 NSCLC trial is also positive, the story changes again.
At that point, intismeran starts looking less like a single successful drug and more like a major oncology franchise.
4/And if it eventually works in metastatic disease — meaning cancer that has already spread — or even without Keytruda?
Then the potential becomes much broader.
The biological idea is powerful.
Take a patient’s tumor.
Sequence its DNA.
Find mutations that produce abnormal proteins only the cancer carries.
These abnormal targets are called neoantigens.
Choose a group of those neoantigens.
Encode them into a personalized mRNA treatment.
Inject it.
The goal is to train the patient’s immune system — especially T cells — to recognize and attack the cancer.
Keytruda can then help by removing one of the “brakes” that normally prevents immune cells from attacking strongly enough.
If this process works repeatedly across different cancers, Moderna may have built something much harder to reproduce than a normal drug:
An entire personalized oncology platform.
Tumor sequencing.
Neoantigen selection.
Individual mRNA design.
Manufacturing.
Quality control.
Delivery to the patient.
And immune activation.
That is the bull case.
But at around $190, part of that bull case is already in the stock.
This is no longer:
“Melanoma works, and every other cancer is free upside.”
It is closer to:
“Melanoma works, and investors are already assigning a meaningful probability that several other cancers will work too.”
That distinction matters enormously.
There is another reason for caution.
We still do not have the complete INTerpath-001 dataset.
The headline tells us the trial succeeded.
But we still need to know how large the benefit actually was.
Important things to look for include:
- The hazard ratio — a measure of how much the treatment reduced the risk of recurrence or metastasis over time;
-The absolute difference in the number of patients who relapsed;
-The recurrence curves showing how the two groups separated over time;
-Whether the benefit was consistent across different patient subgroups;
-The detailed safety profile;
-And OS — overall survival — meaning whether patients ultimately live longer.
The full results are expected at ESMO, the European Society for Medical Oncology congress, on October 24.
So the investment case today depends heavily on which scenario eventually plays out.
Bear case:
The Phase 3 trial is technically positive, but the actual benefit is smaller than investors hoped.
Then kidney, bladder and lung cancer trials disappoint.
At today’s valuation, that could be painful.
Base case:
The melanoma data are strong.
Intismeran also works in one or two additional cancers.
Moderna becomes an important oncology company.
But a meaningful part of that success may already be reflected in the current stock price.
Bull case:
The signal repeats across kidney cancer, bladder cancer and especially lung cancer.
Intismeran becomes a true multi-cancer franchise.
At that point, eventual global sales of $15–20B or more become much easier to imagine.
Extreme bull case:
The technology works across many different tumor types.
It also works outside the post-surgery setting.
And the success of intismeran helps validate other cancer programs inside Moderna.
Then we are no longer valuing a single drug.
We are valuing a reusable personalized cancer-treatment platform.
That could be much bigger.
But we are nowhere near proving that yet.
Ok, let’s try to sum it up:
Remember the four layers we talked about earlier?
Scientifically, Moderna is still at Layer 1.
But at today’s market cap, the stock already seems to price in much of Layer 2 — with some optionality on Layer 3.
If Moderna can fully validate Layer 3, and eventually reach Layer 4, that could open the door to another major rerating.
That’s the upside I’d be buying into today — not melanoma alone.
The September post explicitly discussed what appeared to be priced in. Our view was that the valuation already reflected expectations beyond melanoma, including success in other cancers.
Exceptional melanoma data would strengthen confidence in the broader platform and could justify assigning more value to the other programs. That could support further upside, but it would increase confidence rather than prove efficacy across cancers. Lung, kidney and bladder cancer still require their own clinical evidence.
‘Bullish’ here means strengthening that broader thesis; it does not automatically mean the shares are undervalued.
Fair point that we did not quantify each scenario. A detailed valuation model would make the connection clearer, but assigning a share-price target to each hazard ratio alone would imply a precision the data cannot support.

What Happened in the Biotech World Today?
Biotech Explained — Wednesday, October 21, 2026
$ROIV $RHHBY $KYTX $SGIOY
The five stories that mattered today — and what they could change for patients.
Market temperature and Macro
The XBI closed at $152.76 (−0.31%), while the SOXX rose 2.11% to $510.54. The Nasdaq Composite gained 1.05%. The rebound in AI-related stocks did not extend across biotech.
SpaceX’s results and Super Micro’s stronger preliminary outlook supported sentiment around AI infrastructure. Prospects for renewed Russia–Ukraine negotiations were another macro development and may have helped temper oil prices; Brent nevertheless settled almost unchanged at $108.82.
1. $ROIV — Two Phase 3 successes in non-infectious uveitis
Brepocitinib met the primary and all key secondary endpoints in both CLARITY trials. The hazard ratios for treatment failure versus placebo were 0.27 and 0.22. Across the studies, 62% of treated patients achieved steroid-free disease quiescence at Week 48, versus 20% on placebo.
Why it matters: Two independent randomized trials support a possible way to control sight-threatening inflammation while reducing reliance on steroids. A regulatory submission is planned for the first half of 2027. Comparisons with other active treatments remain an open question.
2. $RHHBY — Fenebrutinib adds new evidence in multiple sclerosis
Roche presented further Phase 3 analyses of its oral BTK inhibitor. Pooled data showed 55% fewer annualized relapses than with teriflunomide in relapsing MS. New imaging analyses found up to 30% less slowly expanding lesion volume across the studied MS populations versus active comparators.
Why it matters: The imaging result suggests an effect on chronic brain inflammation, a difficult target in MS. It does not yet prove that patients will experience less long-term disability because of that imaging change. The relapse results were known before today, and liver safety remains important to assess.
3. $KYTX — A CAR-T response persists at one year in stiff person syndrome
Kyverna presented the full one-year KYSA-8 data set for miv-cel. Among 26 patients, median time to complete the 25-foot walk improved 49% from baseline. 20/26 achieved a clinically meaningful improvement, and 24/26 remained free of chronic immunotherapy for the condition.
Why it matters: Sustained improvement after one treatment is a striking signal in a severely disabling autoimmune disease. The trial has no comparison arm, so the size of the treatment effect remains uncertain. There were no high-grade cases of cytokine release syndrome or ICANS, but five patients had grade 3/4 neutropenia. Initial one-year results had been announced in September; today’s presentation adds the fuller data set.
4. Shionogi / F2G — More Phase 3 detail for oral olorofim
In the 225-patient OASIS trial, Day 42 mortality was 23.8% with olorofim versus 24.3% with liposomal amphotericin B followed by standard care. The study met its prespecified non-inferiority criterion in patients for whom standard azole antifungals were unsuitable or ineffective.
Why it matters: An oral treatment with a new antifungal mechanism could expand options for patients with invasive aspergillosis. Renal adverse events attributed to treatment were less frequent with olorofim. The non-inferiority margin was 20 percentage points, liver safety still needs attention, and the positive primary result was already announced in June; today brings more detailed data.
5. Lumen Bioscience — An early signal against C. difficile recurrence
In the randomized portion of RePreve, 10/12 patients (83.3%) receiving LMN-201 plus standard antibiotics achieved global cure, versus 4/11 (36.4%) receiving placebo plus antibiotics. Global cure meant successful treatment of the initial infection without recurrence.
Why it matters: The oral biologic offers an interesting approach to preventing a difficult infection from returning. The difference is promising, but the decisive randomized comparison included only 23 patients. Larger trials must establish whether it holds up.
News of the day — $ROIV
Brepocitinib is today’s editorial choice: two positive Phase 3 trials, replicated effects across clinical endpoints, and a potential reduction in long-term steroid use for patients with uveitis.
Science first. Investment second.
A few additions to our UEG Week 2026 IBD recap :
Abivax — $ABVX
Thanks to @A_May_MD for highlighting the obefazimod endoscopic findings that prompted this closer look.
In ABTECT maintenance, strict endoscopic remission — Mayo endoscopic subscore of 0 — reached:
• 41.5% with 25 mg daily.
• 47.7% with 50 mg.
• 9.9% with placebo.
Among patients with prior inadequate response, loss of response or intolerance to advanced therapies, the 50 mg rate was 38.9% versus 5.6%.
These findings deserve greater weight in our recap and strengthen the efficacy case in previously treated patients.
The maintenance population consisted of responders to obefazimod induction. Cross-trial comparisons with other medicines remain indirect and cannot establish superiority.
Gilead — $GILD
A population detail worth adding: SWIFT excluded patients previously exposed to vedolizumab or another integrin antagonist.
Its results therefore do not establish efficacy after failure of an anti-integrin treatment.
Odyssey Therapeutics — $ODTX
The OD-001 Phase 2a efficacy analysis included 56 evaluable patients who completed induction, out of 61 treated. The pooled 20 mg twice-daily results cited in our recap concerned 41 evaluable patients.
This was an open-label study without a concurrent placebo group. The findings remain an early clinical signal requiring controlled confirmation.
The main addition is greater recognition of obefazimod’s endoscopic efficacy. The Gilead and Odyssey details clarify the populations behind the reported results.
UEG Week 2026 — Congress Recap: Key Data in Ulcerative Colitis and Crohn’s Disease
The European gastroenterology congress took place in Barcelona from October 17–20.
In inflammatory bowel disease — IBD — presentations covered new oral treatments, combination therapies, fibrosis and treatment targets.
Here are the main findings, with the context needed to interpret them.
1 . Gilead: positive Phase 2 results for a new oral treatment
Emvistegrast ($GILD) inhibits α4β7 integrin, which helps certain immune cells move into the intestine.
In SWIFT, the 200 mg daily dose achieved the following at Week 12:
• Clinical response: 66% versus 37% with placebo.
• Clinical remission: 31% versus 8%.
• Endoscopic improvement: 39% versus 12%.
Only 22% of participants had previously failed one or two advanced therapies. Maintenance results are still pending. Gilead plans to initiate Phase 3 in 2027.
2 .Abivax: Phase 3 maintenance data for obefazimod
Obefazimod ($ABVX) is an oral treatment that increases the expression of miR-124, a microRNA involved in regulating inflammation.
In ABTECT, clinical remission after 44 weeks of maintenance treatment reached:
• 50.8% with 25 mg daily.
• 51.3% with 50 mg.
• 10.4% with placebo.
These headline results had already been announced in June. They concern patients who responded to eight weeks of induction treatment and were then re-randomized to active treatment or placebo.
They therefore cannot be directly compared with SWIFT’s induction results. Abivax plans to submit its US regulatory application in late 2026.
3 . Odyssey: an early clinical signal with OD-001
OD-001 is an oral inhibitor targeting the scaffolding function of RIPK2, a protein involved in immune signaling.
In a Phase 2a ulcerative colitis study, the pooled 20 mg twice-daily group achieved the following at Week 12:
• Clinical response: 61%.
• Clinical remission: 27%.
This group included 41 efficacy-evaluable patients. The trial was open-label, with no concurrent placebo control. The findings support further development, but do not establish how much of the improvement was attributable to treatment. Controlled trials are planned.
4 . Sanofi/Teva: longer-term results for duvakitug
Duvakitug is an anti-TL1A antibody targeting a pathway involved in inflammation and potentially fibrosis.
The RELIEVE UCCD extension included 130 patients who had responded to 14 weeks of induction treatment.
After an additional 44 weeks of maintenance, the 900 mg dose administered every four weeks was associated with:
• 58% clinical remission in ulcerative colitis.
• 55% endoscopic response in Crohn’s disease.
The extension compared two active doses, without a placebo group. Biomarker analyses explored inflammation and tissue remodeling; they do not demonstrate clinical prevention of fibrosis.
5 .AbbVie: combining two mechanisms in Crohn’s disease
ABBV-466 ($ABBV) combines risankizumab, an IL-23 inhibitor, with trosunilimab, an α4β7 inhibitor.
In TARGET-CD, a randomized, open-label Phase 2a study involving 300 patients, endoscopic remission rates were:
• At Week 12: 30.2% with the combination, versus 21.3% with risankizumab and 18% with trosunilimab.
• At Week 24: 40.5%, versus 22.4% and 20%, respectively.
Three-quarters of participants had failed at least one advanced therapy. This direct comparison reported numerically higher rates with the combination. Longer-term efficacy and safety require further evaluation.
6 .Agomab: investigating treatment for intestinal strictures
Ontunisertib is a gut-restricted oral inhibitor of ALK5, a receptor in the TGF-β pathway.
In the open-label STENOVA extension, 49 patients with fibrostenosing Crohn’s disease received treatment alongside standard care. Thirty-seven completed the 48-week extension.
The findings included signals of radiological stricture stabilization, one endoscopic dilation and no surgery.
The small sample and absence of a control group prevent conclusions about fibrosis reversal or a reduction in interventions.
7. PROFILE: the consequences of treating early
The five-year PROFILE follow-up, already published in September, involved patients with newly diagnosed Crohn’s disease.
Crohn’s-related abdominal surgery occurred in:
• 6 of 193 patients initially assigned to early infliximab plus an immunomodulator.
• 26 of 193 assigned to a step-up treatment strategy.
These findings document the long-term consequences of the initial treatment strategy.
8 .VERDICT and STRIDE-III: which treatment targets should be pursued?
In ulcerative colitis, VERDICT compared three targets: symptomatic remission, symptomatic plus endoscopic remission, and the addition of histological remission.
Among patients who achieved their assigned target, those in the group targeting all three dimensions had a longer time to a complication than those targeting symptoms alone: HR 0.33.
The composite endpoint included the use of rescue therapy. Adding histology did not demonstrate an additional benefit over targeting clinical and endoscopic remission.
STRIDE-III also clarifies that histological remission in ulcerative colitis and transmural remission in Crohn’s disease are adjunctive targets. Failure to achieve them can inform treatment optimization and monitoring, but generally should not trigger a treatment switch on its own.
Other presentations extended the evidence for existing treatments: Johnson & Johnson’s guselkumab, including three-year Crohn’s disease outcomes and perianal fistula data, and Lilly’s mirikizumab in pediatric ulcerative colitis.
Science first. Investment second.

$IRON: Why is Disc Medicine raising $450M?
Disc Medicine has priced a public offering of 6 million new shares at $75 each, for $450 million in gross proceeds. The underwriters may buy another 900,000 shares, which would bring the total to $517.5 million. The offering is expected to close on October 22.
The announcement comes just after positive Phase 3 APOLLO results for bitopertin, Disc’s treatment in development for EPP and XLP, rare diseases in which exposure to light can cause severe pain. Disc plans to use the proceeds for drug development and preparations for a potential commercial launch.
For investors, the benefit is more funding to carry the program through its next steps, the cost is dilution
Disc already reported $717.7 million in cash and investments at June 30, with a projected runway into 2029. The company is using the momentum after APOLLO to strengthen its finances ahead of the regulatory and launch phase.
Science first. Investment second.
Why is $IRON up more than 30% today?
Disc Medicine shares are up more than 30% as I write.
The reason: its lead drug, bitopertin, has delivered positive Phase 3 results in erythropoietic protoporphyria (EPP).
In EPP, a light-sensitive molecule called PPIX accumulates. Exposure to light can trigger severe pain, forcing patients to limit ordinary outdoor activities. Bitopertin is a daily oral drug designed to reduce PPIX production.
The APOLLO trial randomized 183 adults and adolescents to bitopertin or placebo for six months. It met both co-primary endpoints:
• Whole-blood PPIX: 41.4% reduction versus placebo, p<0.0001.
• Time in light without pain during month 6: 12.2 additional hours over the month, p=0.0010.
The rate of phototoxic reactions was also 71% lower than with placebo over six months. Patients spent more time in light and experienced fewer painful reactions.
That clinical result matters because the FDA declined accelerated approval in February. It accepted that bitopertin lowered PPIX, but wanted stronger evidence that patients actually benefited. APOLLO now provides that evidence in a controlled Phase 3 trial. FDA approval is still pending.
Safety deserves attention. Dizziness occurred in 50% of bitopertin patients versus 11% on placebo, generally mild or moderate and transient. Grade 3 adverse events occurred in 7% versus 3%. No patient in the bitopertin group discontinued the study.
Disc plans to resubmit its FDA application by year-end 2026 and anticipates a decision around mid-2027. We still need the detailed patient-level and subgroup results.
Our take: APOLLO substantially strengthens the clinical case for bitopertin. The share-price surge reflects that change in evidence. Whether $IRON is attractive after this move requires a fresh look at valuation.
What Happened in the Biotech World Today?
Biotech Explained — Tuesday, October 20, 2026
$AZN $AMGN $RCUS $RAPP $ODTX
The five stories that mattered today — and what they could change for patients.
Market temperature & macro
The XBI closed at $153.24 (+3.64%), while the SOXX finished at $500.01 (−0.38%). The Nasdaq Composite slipped 0.02%, highlighting biotech’s strong outperformance in a session that favored healthcare.
Markets reacted to reports of progress in U.S.–Iran negotiations before more cautious comments tempered the optimism. Oil swung sharply: Brent settled at $108.79, after approaching $112. Lower Treasury yields and weak housing data also shaped the session.
After hours, AI infrastructure received two complementary signals: SMCI reported preliminary quarterly revenue of $18.5–19.5 billion, well above its previous forecast, citing stronger AI platform demand. Meanwhile, SpaceX reported $19.6 billion in Q3 capital spending, including $16.4 billion in AI, and its CFO projected $35–40 billion of total capex in Q4.
Together, stronger server sales and plans for additional computing capacity offered support for semiconductor demand. At 5:28 p.m. ET, the SOXX traded at $509.09, up 1.82% after hours. NVIDIA’s October 21 GTC Berlin keynote is another event to watch.
1. $AZN / $AMGN — Tezspire: detailed CROSSING results in eosinophilic esophagitis
At week 24, histologic remission reached 64.8% and 68.9%, depending on dose, versus 3.5% with placebo, alongside improvements in swallowing difficulties. Benefits were sustained through week 52.
Why it matters: This Phase 3 trial strengthens the case for targeting TSLP with treatment administered every four weeks. The overall trial success was announced in August; today’s development is the detailed presentation. No head-to-head trial establishes superiority over Dupixent.
2. $RCUS — Casdatifan: a growing clinical dataset in kidney cancer
In ARC-20, casdatifan plus cabozantinib achieved median progression-free survival of 14.5 months in second-line treatment, among 43 evaluable patients. Pooled monotherapy cohorts reported median overall survival of 35 months in 121 heavily pretreated patients.
Why it matters: These findings support developing this HIF-2α inhibitor across several treatment settings. However, the cohorts lack randomized controls. Phase 3 PEAK-1 must establish whether adding casdatifan improves outcomes over cabozantinib alone.
3. $RAPP — RAP-219 meets its Phase 2 goal in bipolar mania
In a randomized trial of 253 adults, the YMRS mania score fell by 15.4 points, versus 12.3 points with placebo, at three weeks: a 3.1-point difference, p=0.045. No serious adverse events were reported during this treatment period.
Why it matters: Targeting TARPγ8-associated AMPA receptors could offer a new approach in psychiatry. The result is statistically positive, but the additional benefit is modest and follow-up is short. Phase 3 trials are expected to begin in the second half of 2027.
4. Nano24 — An encouraging signal for inhaled exosomes in ARDS
In a randomized trial of 90 patients, Nano24 reported that 85% of patients receiving EXO-CD24 were free of respiratory failure at day 7, versus 33.3% with placebo. Reported six-month mortality was 13.3% versus 43.3%.
Why it matters: A confirmed survival benefit in acute respiratory distress syndrome would be a major advance. These remain company-reported findings from a small trial; the protocol and respiratory endpoint require closer examination. A larger Phase 3 trial is planned.
5. $ODTX — OD-001: additional evidence for an oral approach to ulcerative colitis
The completed induction analysis reported 27% clinical remission and 61% clinical response at week 12 among 41 evaluable patients receiving 20 mg twice daily. Biomarker and biopsy analyses provided findings consistent with inhibition of RIPK2 signaling.
Why it matters: This approach could influence several inflammatory pathways and be combined with vedolizumab. But the study was open-label, without placebo: these rates cannot precisely establish the treatment effect or rank OD-001 against other therapies. Controlled trials must provide the next evidence.
Science first. Investment second.
@seahorse_anton Thanks, appreciate it! This took quite a bit of time.
The TAM can be much larger then 550m but several steps remain before that potential translztes into clinical proof and market share , I ll be watching
After the $RCUS readout: let’s study the business case for casdatifan. Activity is there , what’s already priced in at ~$24? https://t.co/XqKiLQHFh1
What a day in biotech!
Biotech Explained — Monday, October 19, 2026
$IRON $GSK $EYPT $SLN $GILD $OSE $RHHBY $ABBV $CMHC
The news cycle was unusually crowded. Five stories would not do it justice, so today’s recap expands to 10 developments, ranked by their clinical, scientific, and strategic significance.
Market temperature & macro
The XBI closed at $147.83 (+2.43%), while the SOXX finished at $501.93 (+1.73%), ending a nine-session losing streak. The Nasdaq Composite gained 1.11%.
But the closing numbers hide a less convincing session: the SOXX was up roughly 4% at its intraday high before giving back much of that gain.
Early sentiment benefited from falling oil prices after the U.S. eased Russia-related sanctions, alongside a rebound in beaten-down technology stocks.
That relief faded as renewed Middle East supply concerns helped oil recover: Brent finished at $109.55, only slightly lower after dropping more than $3 earlier. Meanwhile, the U.S. 10-year Treasury yield rose to around 5.14%, keeping financing costs in focus.
The VIX eased after Friday’s jump in anxiety. Some investors will hope this marks a bottom, but one rebound cannot establish that.
For biotech, clinical results and deal activity were encouraging; high borrowing costs remain a difficult backdrop.
1. $IRON — Bitopertin succeeds in Phase 3 for protoporphyria
In APOLLO, bitopertin met both co-primary endpoints: a 41.4% reduction in PPIX versus placebo and 12.2 additional hours per month of pain-free light exposure at month 6. The trial connects a biological effect with something patients can experience in daily life.
It is today’s strongest clinical readout, particularly following Disc Medicine’s earlier FDA setback. These are still topline results; the company plans to resubmit its application in late 2026.
2. $GSK — Jemperli receives accelerated FDA approval in rectal cancer
The FDA approved dostarlimab for certain patients with locally advanced dMMR/MSI-H rectal cancer. For some patients, the treatment may offer a path that avoids chemotherapy, radiation, and surgery. That is a substantial potential change in care. The approval is based on interim results from a single-arm study, so longer follow-up remains essential to establish how durable the responses are.
3. $EYPT — A second Phase 3 failure for DURAVYU in wet AMD
LUCIA failed to establish visual noninferiority to aflibercept, following the earlier LUGANO failure. Fewer injections could not make up for missing the primary endpoint. EyePoint is ending development in wet AMD, shifting its focus to diabetic macular edema, and reducing its workforce by approximately 53%. It is a stark reminder that convenience must come with sufficient efficacy.
4. $SLN — Merck KGaA moves to acquire Silence Therapeutics
The agreement values Silence’s equity at approximately $1.3 billion, offering $20 per ADS, a premium of roughly 130% to its previous closing price. Merck would gain divesiran, an siRNA candidate for polycythemia vera, and Silence’s RNA platform. The deal is a strong financial signal for RNA therapeutics and rare-disease biotech. Closing remains subject to conditions.
5. $GILD — An oral α4β7 inhibitor advances in ulcerative colitis
In the Phase 2 SWIFT study, 66% of patients receiving emvistegrast 200 mg achieved a clinical response at week 12, versus 37% on placebo. Clinical remission was 31% versus 8%. The α4β7 pathway is already validated by vedolizumab; Gilead is testing whether it can be targeted with a once-daily oral treatment. The induction signal is encouraging, while maintenance efficacy and its place among existing therapies remain open questions.
6. $OSE, Immunotherapeutics — A survival signal for Tedopi in lung cancer
In the randomized Combi-TED trial, reported one-year survival was 47% with Tedopi plus docetaxel and 46% with Tedopi plus nivolumab, versus 29% with docetaxel alone. This is an interesting signal for patients whose cancer has progressed after prior treatment. But the trial was open-label and included 106 patients. The fuller ESMO presentation will matter for interpreting the result.
7. $RHHBY/ Hansoh — A broad ADC partnership
Roche will pay $400 million upfront and $110 million on the first anniversary under an agreement covering several antibody-drug conjugates. Future milestone payments could exceed $15 billion, but those payments are conditional. The deal illustrates major pharmaceutical companies’ continued interest in ADC platforms developed in China. The initial programs are still at an early stage.
8. TLC BioSciences — Positive Phase 3 results in postoperative pain
TLC590 significantly reduced pain over 72 hours versus placebo (p=0.0024) and lowered rescue opioid use. The company also reported favorable results against liposomal bupivacaine. This could become a useful option for pain control after surgery. The absolute size of the benefit and the regulatory path deserve closer examination.
9. $ABBV — Dual targeting shows promise in Crohn’s disease
At week 24, 40.5% of patients receiving ABBV-466 achieved endoscopic remission, versus 22.4% on risankizumab and 20.0% on trosunilimab in this Phase 2a study. Targeting both IL-23 and α4β7 may improve control of intestinal inflammation. The concept is compelling, but this was an open-label, exploratory study. It does not yet establish definitive superiority over existing treatments.
10. $CMHC — A locally acting rifamycin shows promise in distal ulcerative colitis
In a small Phase 2 study, clinical remission at six weeks reached 46.2% with a 1% rifamycin gel, versus 14.3% with placebo. The approach aims to treat distal ulcerative colitis and ulcerative proctitis locally, without systemic immunosuppression. It offers a distinct therapeutic idea, but the primary analysis included 60 of the 76 enrolled patients. Larger studies are needed to confirm the finding.
The day in perspective:
IRON delivered the most compelling clinical readout, while GSK reached a regulatory milestone that could already change care for selected patients.
EyePoint showed how much can hinge on a pivotal endpoint. Merck/Silence and Roche/Hansoh, meanwhile, showed that strategic investment in biotech continues.
An extraordinary day for company news, against a macro backdrop that remains unsettled.
@RosenzweigBrian Yes you’re right , thanks for flagging $IOVA.
I missed the final Phase 2 lifileucel + pembrolizumab melanoma data scheduled for Friday, October 23.
The sarcoma pilot results follow on Monday, October 26, just outside this weekly window. Both deserve coverage.
BIG WEEK AHEAD IN BIOTECH — OCTOBER 19–25
From inflammatory bowel disease to CAR-T therapies, kidney disease and cancer, several important clinical updates are on the calendar.
Some will bring first clinical results. Others will reveal the details behind already positive headline announcements.
Here is what to watch, and what each event could actually tell us.
Before the week starts: ESMO abstracts
Regular ESMO abstracts were scheduled for release on Sunday, October 18, at 6:05 p.m. ET.
That is separate from the conference opening on October 23. Late-breaking abstracts follow their own release schedules, and presentations may contain more recent data than the initial abstracts.
MONDAY, OCTOBER 19
$GILD — Emvistegrast: an oral treatment for ulcerative colitis
The Phase 2 SWIFT study evaluates emvistegrast, previously called GS-1427, in moderate-to-severe ulcerative colitis.
It targets α4β7, a pathway involved in directing immune cells toward the gut. The appeal is an oral approach to a mechanism already used by an established injectable treatment.
The primary endpoint is clinical response at 12 weeks: a meaningful improvement in disease activity. Clinical remission is a stricter outcome and will also attract considerable attention.
What matters: the difference versus placebo, endoscopic improvement, dose response and safety.
For $ABVX followers, this is also a relevant competitive update. However, results from different trials cannot establish which drug is better without accounting for differences in patients, treatment duration and endpoint definitions. The presentation is scheduled for Monday at UEG Week.
$ABVX — Obefazimod: looking beyond induction
The Phase 3 ABTECT maintenance presentation is scheduled for 4:50 a.m. ET on Monday, followed by an analysis according to prior advanced therapy exposure on Tuesday at 6:20 a.m. ET.
Induction asks whether a drug can bring active disease under control. Maintenance asks whether that benefit can last.
The positive maintenance headline is already public. The focus now is the detailed evidence: sustained remission, healing seen on endoscopy, safety and outcomes in patients who previously failed other advanced treatments.
TUESDAY, OCTOBER 20
$RCUS — Casdatifan: expanding its role in kidney cancer
Investor event: 8:30 a.m. ET.
Arcus will present new ARC-20 data for casdatifan, an oral HIF-2α inhibitor being developed for clear cell kidney cancer.
The update spans several treatment settings:
First-line combinations with immunotherapy.
A second-line combination with cabozantinib.
Later-line treatment with casdatifan alone.
The question is whether activity remains convincing across these settings, with sufficient durability and manageable toxicity.
Watch response rates, early disease progression, progression-free survival and treatment discontinuations. These are early, nonrandomized cohorts, so comparisons with other treatments remain exploratory. This is an ARC-20 update; the randomized Phase 3 PEAK-1 results are a separate event.
WEDNESDAY, OCTOBER 21 → FRIDAY, OCTOBER 23
$TGTX / $KYTX — CAR-T therapies move further into neurology
At MSToronto2026, both companies will present programs using CAR-T cells to target B cells involved in autoimmune disease.
$TGTX — Azer-cel
Late-breaking abstracts become available on Wednesday, October 21.
The oral presentation of initial Phase 1 results in primary progressive multiple sclerosis is scheduled for Friday, October 23, at 10:45 a.m. ET.
Azer-cel is an allogeneic CAR-T therapy, meaning its cells come from a donor.
The first questions concern feasibility, safety, B-cell depletion and early clinical observations. Patient numbers and follow-up will determine how much can reasonably be concluded.
$KYTX — Miv-cel
Kyverna will present full one-year results in stiff-person syndrome on Wednesday, followed by updated progressive multiple sclerosis data on Friday at 11:05 a.m. ET.
Miv-cel uses the patient’s own cells.
For stiff-person syndrome, the headline findings have already been announced. The presentation should help assess the durability of benefit, functional improvement and safety. In progressive MS, the depth and duration of follow-up will be especially important.
THURSDAY, OCTOBER 22
$VERA / RemeGen (https://t.co/KQDTk0bEcl) — Longer-term evidence in IgA nephropathy
IgA nephropathy is a disease in which immune deposits damage the kidneys. Reducing protein leakage into urine is useful, but preserving kidney function over time is the central clinical goal.
$VERA: the final efficacy analysis from Phase 3 ORIGIN 3 will be presented at 11:05 a.m. ET. The headline findings have already been announced; the presentation will provide the detailed longer-term evidence for atacicept.
RemeGen: final two-year Phase 3 TELIGAN results for telitacicept are also scheduled for Thursday.
Both programs target BAFF/APRIL signaling, which supports B cells involved in the disease. Watch kidney function, proteinuria, safety and consistency across patient groups.
$HUMA — Improving dialysis access for women
Presentation: 6:40 p.m. ET.
The randomized Phase 3 V012 study compares Humacyte’s bioengineered vessel with a standard arteriovenous fistula in women requiring hemodialysis access.
The practical question: can the vessel provide usable access while reducing dependence on dialysis catheters?
Positive interim results were announced in June. Thursday is a conference presentation of that evidence, rather than the first announcement of success.
Watch catheter-free time, access usability, complications and durability. Humacyte plans a supplemental FDA application this quarter; that is a submission milestone, separate from an approval decision.
FRIDAY, OCTOBER 23
$IONS / $RHHBY — Sefaxersen in IgA nephropathy
Detailed interim Phase 3 IMAgINATION results will be presented at ASN.
Sefaxersen is an antisense medicine that reduces factor B, part of the complement system involved in immune-mediated kidney injury.
The positive proteinuria headline is already known. Watch the magnitude and consistency of the reduction, alongside safety.
Longer-term kidney function results remain important: reducing proteinuria and demonstrating durable preservation of kidney function are distinct pieces of evidence.
$ALNY — First ALN-HTT02 data in Huntington’s disease
Initial Phase 1 results are scheduled for presentation at EHDN.
ALN-HTT02 is an RNA interference therapy designed to lower huntingtin production.
This is an early study. The key questions concern safety and whether the treatment produces the intended biological effect.
A reduction in huntingtin would support the mechanism. Demonstrating that treatment slows clinical deterioration requires additional evidence and follow-up.
$TNGX / $RVMD — Combining PRMT5 and RAS inhibition
Tango webcast: 7:30 a.m. ET Friday. Updated poster: Sunday, October 25.
Tango will discuss vopimetostat combined with Revolution Medicines’ RAS inhibitors, daraxonrasib or zoldonrasib.
The combinations are being studied in selected pancreatic and lung cancers with MTAP deletion and RAS mutations.
Watch how many patients were treated, which doses they received, confirmed responses, response duration and tolerability. Small early cohorts need careful interpretation, especially when several drugs contribute to both activity and toxicity.
$IMTX — Updates across the PRAME franchise
Immatics will present IMA203CD8 data on Friday, followed by IMA402 on Saturday.
These programs use different approaches to direct immune activity against PRAME-expressing cancer cells.
Watch confirmed responses, durability, activity across tumor types and safety. Findings from the two programs should be assessed separately because their treatment approaches differ.
$ZLAB — Zoci in first-line small cell lung cancer
Webcast: Friday at 8:00 a.m. ET.
Zai Lab will discuss initial global data for its DLL3-targeted antibody-drug conjugate zoci, combined with atezolizumab, with or without chemotherapy, in extensive-stage small cell lung cancer.
Watch response depth, durability, follow-up and the safety of the combinations.
The webcast falls within this week. The ESMO poster presentation is scheduled for Monday, October 26, outside this preview’s main window.
SATURDAY, OCTOBER 24
$MRNA / $MRK — INTerpath-001: the full Phase 3 melanoma data
Presidential Symposium begins at 10:30 a.m. ET. Moderna webcast: 1:00 p.m. ET.
The study evaluates individualized neoantigen therapy intismeran autogene, previously mRNA-4157, plus Keytruda versus Keytruda alone after surgery for stage IIB–IV melanoma.
The positive headline has already been announced.
The full presentation should show the size of the recurrence-free survival benefit, the effect on distant metastases, consistency across patient groups and added toxicity.
For patients treated after surgery, the central question is how much additional protection the combination provides against cancer returning.
$VERA — PIONEER: testing atacicept beyond the pivotal study
Presentation: 7:30 p.m. ET.
Interim Phase 2 results will cover adult IgA nephropathy and primary membranous nephropathy cohorts.
Watch patient numbers, proteinuria changes, kidney function and safety. This is a separate study from ORIGIN 3, with earlier evidence in additional patient populations.
SUNDAY, OCTOBER 25
$SMMT — HARMONi-GI1: ivonescimab beyond lung cancer
Presidential Symposium window: 11:30 a.m.–1:15 p.m. ET.
This China-based Phase 3 trial compares ivonescimab plus chemotherapy with durvalumab plus chemotherapy as first-line treatment for advanced biliary tract cancer.
A positive overall survival headline has already been announced, alongside positive secondary endpoints.
The full data should clarify the magnitude of the survival benefit, progression-free survival, safety and consistency across patient groups.
How well the findings apply beyond the Chinese study population will also matter. This presentation is separate from Summit’s U.S. lung cancer regulatory application.
$GSK — AZUR-1: durable complete responses in rectal cancer
Presentation session: 5:15–6:45 a.m. ET.
The registrational Phase 2 study evaluates dostarlimab in locally advanced rectal cancers with dMMR/MSI-H biology.
The clinical interest is whether immunotherapy can produce durable complete responses and support an approach that preserves the organ.
Watch sustained response rates, follow-up, subsequent treatment and safety. GSK has already signaled positive findings ahead of the presentation.
$HCM / $AZN — SAFFRON: targeting a mechanism of resistance
Phase 3 SAFFRON evaluates savolitinib plus osimertinib in EGFR-mutated lung cancer with MET overexpression and/or amplification after progression on osimertinib.
MET signaling can provide a route around EGFR inhibition. The combination aims to block that escape mechanism.
The positive headline is already known. Watch the size and durability of benefit, safety and outcomes according to the biomarker used to select patients.
FDA DATES JUST AFTER THIS WEEK
Monday, October 26 — $GSK / $IONS: bepirovirsen in chronic hepatitis B.
Friday, October 30 — $INO: INO-3107 in recurrent respiratory papillomatosis.
These are FDA target action dates, rather than clinical presentation dates. Decisions can occur earlier, and review timelines can be extended.
A busy week, with very different levels of evidence.
Science first. Investment second.
$GILD, $TAK, $ABVX — Three approaches to ulcerative colitis. What do today’s data change?
Two studies presented on October 19 address different questions in ulcerative colitis, or UC.
SWIFT tests Gilead’s experimental oral drug.
VERDICT examines how deeply physicians should aim to control the disease using treatment strategies based on Takeda’s Entyvio.
Abivax is developing a third drug, obefazimod, whose results are relevant to both discussions.
To understand the competitive landscape, we first need to distinguish the three treatments.
1/ Takeda: Entyvio, the established treatment
Entyvio, or vedolizumab, is an antibody that blocks the α4β7 integrin. This protein helps certain immune cells reach the intestine. Blocking that recruitment reduces intestinal inflammation.
Entyvio is already approved and administered through intravenous infusion or subcutaneous injection.
Its contribution: a clinically validated target, supported by substantial treatment experience. It provides an established reference for this biological approach.
2/ Gilead: emvistegrast, targeting α4β7 with a daily pill
Emvistegrast targets the same integrin as Entyvio, but through an oral small molecule.
It is a different drug, and its efficacy and safety must be demonstrated independently.
Its potential contribution: delivering this treatment approach orally, with durable disease control.
Convenience matters, but the competitive question is broader: can it deliver a compelling balance of efficacy, safety and ease of use?
3/ Abivax: obefazimod, an oral drug with a different mechanism
Obefazimod does not target α4β7. It increases the expression of miR-124, a microRNA involved in regulating inflammation.
Its potential contribution: another way to control UC with a once-daily oral treatment, supported by Phase 3 induction and maintenance results.
Both emvistegrast and obefazimod remain investigational. Obefazimod is further advanced in clinical development.
What did Gilead’s new study show?
SWIFT is a randomized, placebo-controlled Phase 2 trial.
At Week 12, the 200 mg dose achieved:
• Clinical response: 66% versus 37% with placebo.
• Clinical remission: 31% versus 8%.
• Endoscopic improvement: 39% versus 12%.
Clinical response was the primary endpoint. Remission and the other measures were secondary endpoints, with reported nominal p-values.
The results support continued development. Maintenance data are still pending, and Gilead plans to initiate Phase 3 in 2027.
The patient population also matters.
According to Gilead, 78% of participants were advanced-therapy naïve or had not failed an advanced therapy. This does not mean that all 78% were treatment naïve.
Previous exposure to Entyvio or another integrin antagonist was excluded.
Efficacy after Entyvio failure therefore remains an open question. These eligibility criteria also matter when comparing SWIFT with other trials.
Why are Abivax investors watching?
Another effective oral UC treatment could eventually become a commercial competitor.
Obefazimod already has positive Phase 3 results. Among patients who responded to induction and were then re-randomized for 44 weeks of maintenance, the 50 mg dose achieved:
• Clinical remission: 51.3% versus 10.4% with placebo.
• Strict endoscopic remission: 47.7% versus 9.9%.
The endoscopic result is a notable strength of the dataset.
It does not establish superiority over emvistegrast: this compares Phase 3 maintenance with Phase 2 induction, in different patient populations.
What does VERDICT add?
VERDICT asks a different question:
What treatment target should physicians pursue to reduce UC-related complications?
The trial uses vedolizumab-based treatment strategies and compares three corticosteroid-free targets:
• No rectal bleeding.
• The same target plus endoscopic remission, defined as a Mayo endoscopic score ≤1.
• The same targets plus histologic remission, assessed under the microscope in intestinal biopsies.
The primary result was positive : among patients who achieved their assigned target, those in the deepest-remission group had a longer time to their first UC-related complication than those in the symptom-target group.
The hazard ratio was 0.33, with a 95% confidence interval of 0.17–0.67.
Two qualifications matter:
• This analysis concerned patients who achieved their target: 400 of the 672 randomized participants. That analysis was prespecified.
• The composite endpoint included rescue treatment, the most frequent category of complication. The result cannot be translated into “67% fewer hospitalizations or surgeries.”
The contribution is additional prospective evidence supporting objective, deeper disease control beyond symptoms.
Endoscopic healing was already a recognized treatment target. VERDICT strengthens the evidence behind that approach.
Is this particularly good news for Abivax?
It strengthens interest in obefazimod’s endoscopic results.
But VERDICT did not test obefazimod and does not establish its superiority.
There is also an important difference in definitions:
VERDICT’s endoscopic target allows a Mayo score of 0 or 1. The strict endoscopic remission endpoint highlighted for obefazimod requires 0.
And “clinical remission” in ABTECT already includes an endoscopic requirement. It does not simply mean that a patient reports no symptoms.
How should we assess this competition?
Three questions should guide the comparison:
• How deeply does each treatment control the disease? Symptoms, mucosal healing, freedom from corticosteroids and prevention of complications all matter.
• In which patients, and for how long? Initial improvement must be assessed alongside previous treatment failures and durability.
• With what treatment burden? Safety, monitoring, administration and the ability to remain on therapy are essential.
Takeda brings an established treatment and new evidence about treatment targets.
Gilead brings encouraging proof of concept for an oral α4β7 inhibitor.
Abivax brings a more advanced oral program, a different mechanism and notable endoscopic results.
Today’s data advance the discussion. They leave open which treatment will offer the best balance for each patient profile.
$RCUS — Before the Readout: What We Are Watching in Casdatifan
Today, we apply our analytical framework to Arcus Biosciences ahead of its October 20 data presentation.
Arcus is developing casdatifan for clear cell kidney cancer. On Tuesday at 8:30 a.m. New York time, it will present results from several patient groups. These are early results from relatively small groups of patients. They can strengthen the case for casdatifan, but they cannot yet establish how it compares with other treatments.
Here is what matters — from the science to the investment decision.
1/ WHAT IS CASDATIFAN?
In many clear cell kidney cancers, a protein called HIF‑2α remains abnormally active. It helps switch on genes involved in tumor growth and the formation of new blood vessels.
Casdatifan is an oral drug designed to block HIF‑2α. It is a targeted therapy, not an immunotherapy.
Arcus is also testing it with immunotherapies. Zimberelimab blocks PD‑1; ipilimumab blocks CTLA‑4. These are two immune “brakes” discussed in our recent Science piece, “Understanding the Immune Brake.”
The idea is to act on both tumor biology and the immune response. Clinical results must show whether that actually benefits patients, and whether they can tolerate the combination.
2/ WHAT WILL ARCUS PRESENT?
A “line of treatment” tells us where a drug fits in a patient’s care. First line is the first treatment for advanced disease. Later lines follow previous treatments.
Tuesday’s ARC‑20 presentation covers four groups:
• First line: casdatifan + zimberelimab. About 30 patients, with roughly 11 months of median follow-up. We expect safety and early efficacy data, including responses and progression-free survival.
• First line: casdatifan + zimberelimab + ipilimumab. About 20 patients. The focus will be safety and how often disease progresses immediately.
• Second line, after immunotherapy: casdatifan + cabozantinib. Forty-three patients, with roughly 21 months of median follow-up. We expect responses, progression-free survival, early overall survival and safety data.
• Later lines: casdatifan alone. An updated analysis of 121 patients, with roughly 28 months of median follow-up, including survival data.
Why two first-line groups? Arcus is testing two different combinations at the same stage of disease: one adds a single immunotherapy, while the other adds two. The three-drug group is earlier in its development. These groups cannot directly tell us which regimen is better.
Tuesday will also NOT bring the Phase 3 PEAK‑1 result.
PEAK‑1 compares casdatifan + cabozantinib with placebo + cabozantinib. It is designed to answer the stronger question: what benefit does adding casdatifan actually provide?
A separate planned first-line Phase 3 trial, PEAK‑20, would use casdatifan + nivolumab + ipilimumab. Nivolumab is a different PD‑1 inhibitor from the zimberelimab used in the early ARC‑20 group.
3/ WHAT DO WE ALREADY KNOW?
The previously reported results for casdatifan ALONE came from patients who had already received other cancer treatments. They are not results from the first-line combinations we will hear about on Tuesday.
Those patients had received a median of three prior treatments. All had previously received a PD‑1 or PD‑L1 immunotherapy and a drug targeting tumor blood vessels.
“Casdatifan alone” means without another cancer drug given at the same time. It does not mean it was their first cancer treatment.
At the January 30, 2026, data cutoff, the 100 mg once-daily group had:
• 14 confirmed responses among 31 patients assessed for efficacy — about 45%;
• 12 patients with stable disease;
• 5 patients whose disease progressed;
• median progression-free survival of 15.1 months, with median follow-up of 17.9 months.
A response means the tumor shrank enough to meet defined imaging criteria. Stable disease means it had neither shrunk enough to count as a response nor progressed at that assessment. A response is not a cure.
Progression-free survival, or PFS, measures the time until the cancer progresses or the patient dies. Its median does not describe what happened to every individual patient.
Across groups receiving different casdatifan doses, Arcus reported 42 responses among 121 patients — about 35% — and median PFS of 12.2 months.
The 31 patients in the 100 mg group are included in those 121. These are overlapping analyses, not two independent confirmations.
4/ HOW DOES IT COMPARE WITH OTHER DRUGS?
Belzutifan ($MRK) is casdatifan’s most direct competitor. It is already approved, and both drugs block the same target: HIF‑2α.
Arcus is trying to differentiate casdatifan through the drug’s properties. It was designed to penetrate tissues more effectively and produce deeper inhibition of the HIF‑2α pathway. A fall in blood erythropoietin, or EPO, is one measure of that biological effect.
But stronger pathway inhibition does not automatically mean better outcomes for patients. The same action can also contribute to anemia. Efficacy and tolerability both matter.
In a separate randomized trial, belzutifan alone produced responses in 21.9% of previously treated patients. The 45% observed in 31 patients taking casdatifan at 100 mg is numerically higher.
That is an interesting signal, not proof of superiority. The patients and trial designs differed, and 31 patients give an imprecise estimate.
Other drugs in this story act on different parts of the disease. Cabozantinib and lenvatinib block signals involved in blood-vessel formation and tumor growth. Zimberelimab, nivolumab and ipilimumab act on immune checkpoints.
Combining treatments could improve efficacy. It could also add toxicity. Clinical comparisons must establish the balance.
For context, belzutifan + lenvatinib produced a 53% response rate in a randomized trial involving a different patient population. That result comes from two drugs given together. It cannot be directly compared with casdatifan alone.
5/ WHAT CAN TUESDAY ADD?
Casdatifan + cabozantinib has already shown early activity. In June 2025, Arcus reported 11 confirmed responses among 24 patients assessed for efficacy — about 46%. Follow-up was short.
Now we need to know what happens with more time:
Do responses last? How many patients remain free of progression? Can patients stay on the combination?
We must also check who is included in each analysis. A change in the number of patients assessed can move the response percentage without showing that the treatment itself has improved or worsened.
The casdatifan publication in Nature links HIF‑2α inhibition, biological markers and patient outcomes. It comes from the same ARC‑20 program, so it is not an independent replication. A biological correlation supports the mechanism; it does not establish clinical superiority over belzutifan.
Safety deserves equal attention. In earlier data, among 32 patients assessed for safety at 100 mg, treatment-related grade 3 or higher anemia occurred in 25% and hypoxia in 9%.
In the combinations, we will also examine dose reductions, treatment interruptions and permanent discontinuations.
6/ CASH, PRICE AND COMPETITION
As of June 30, Arcus held $775 million in cash and investments, with $100 million of debt drawn. It used $257 million in operating cash during the first half of 2026 — an historical average of about $43 million a month.
Dividing June cash by that historical rate suggests roughly 18 months of funding if spending remained unchanged. That is a mechanical calculation, not a forecast.
Management expects about $600 million in cash at year-end 2026 and believes its business plan is funded until at least the second half of 2028.
The cash gives Arcus time to develop casdatifan. It does not eliminate future financing or dilution risk.
At Friday’s $22.12 close, using approximately 127.3 million shares reported at the end of July, indicative market capitalization is near $2.8 billion. Adding drawn debt and subtracting June cash gives an illustrative enterprise value of about $2.1 billion.
These figures mix different dates. Arcus also has other programs, including quemliclustat in pancreatic cancer and an immunology portfolio. The entire valuation cannot be assigned to casdatifan.
RCUS fell approximately 9.4% between the October 9 and October 16 closes. That is the move over the week.
On Friday alone, RCUS declined about 3.1%, while XBI, a broad biotech fund, fell about 3.6%. Friday’s decline took place during a difficult session for biotech. It does not reveal what Tuesday’s casdatifan data will show.
Competition raises the standard. In September, the FDA approved belzutifan + lenvatinib following PD‑1 or PD‑L1 treatment in advanced kidney cancer with a clear cell component.
In its randomized trial, median PFS was 14.6 months versus 10.6 months with cabozantinib. The final overall-survival analysis was not statistically significant.
Those 14.6 months cannot be directly compared with ARC‑20 to declare a winner. Patient populations, prior treatments and trial designs differ.
7/ OUR CHECKLIST FOR TUESDAY
Six questions will guide our reading:
• How many patients have confirmed responses, and how many progress immediately?
• How long do responses and disease control last?
• Which patient groups appear to benefit?
• Can patients remain on treatment despite anemia, hypoxia and other side effects?
• What do the early first-line combinations really tell us?
• Do the cabozantinib data strengthen the rationale for PEAK‑1?
What could a successful program mean for Arcus?
We can already frame the opportunity. Casdatifan is being developed across several lines of clear cell kidney cancer, so its potential is broader than a single late-line use.
But it is competing in a field where belzutifan is already approved, including in combination regimens.
Casdatifan has not demonstrated superiority over it.
Nor is there an established commercial opportunity for casdatifan beyond kidney cancer today.
Tuesday’s results can change our confidence in the kidney cancer strategy; they cannot settle all three questions.
Possible outcomes:
VERY BULLISH: Durable control with cabozantinib, acceptable safety and convincing early first-line activity. Strong results across treatment lines would raise the probability that casdatifan becomes a major kidney cancer asset for Arcus. We would still need controlled trials to establish its benefit and a credible case for differentiation from competing regimens. Then comes the investment question: how much of that potential is already reflected in the share price?
BULLISH: Strong cabozantinib data, while first-line results remain too early to judge. The most advanced path gains credibility, but the opportunity across multiple treatment lines remains unproven.
MIXED: Interesting first-line signals but disappointing cabozantinib data. The biology remains interesting, while the most advanced development path is weakened.
BASE: Responses are present, but durability, patient subgroups or safety remain difficult to interpret. Uncertainty persists.
BEARISH: Inadequate disease control or toxicity that meaningfully limits treatment. The investment thesis needs to be revisited, even if the stock falls.
These are scenarios for reading the evidence. They are not official success thresholds or predictions of the share-price reaction.
8/ HOW WE APPROACH THE POSITION
Start small before the proof. Build the position as the evidence builds.
Before Tuesday, a small position can express interest in the program for someone willing to accept a substantial loss if the data disappoint. Waiting for the results is also a coherent choice.
For someone already invested, position size should reflect the clinical uncertainty. Believing in the mechanism does not make the outcome predictable.
There is no need to hold a final-sized position before the presentation. Paying a higher price after uncertainty has genuinely fallen can be rational. Buying more solely because the stock has declined does not strengthen the thesis.
After the release, we will ask two separate questions:
Has the program’s probability of success improved?
And does the new share price still offer a reasonable opportunity?
A strong scientific result and a strong investment return are not the same thing.
Even under our Very Bullish scenario, a modest rise in the stock could be disappointing for someone who took substantial risk before the readout. Equally, a large rally could leave little room for error in the next trials. We will assess the data, the remaining risks and the new price together.
Strong data at a sensible price may support a gradual increase.
Good data followed by an excessive rally may call for restraint or profit-taking.
Mixed data require us to identify what remains uncertain. A weaker thesis is not repaired by a lower share price.
Our view before Tuesday: Casdatifan has shown activity in patients who had already received several treatments. The new data should clarify durability, the practicality of its combinations and the strength of the next development steps. The larger opportunity is to earn a meaningful place across several lines of kidney cancer treatment.
Tuesday’s presentation cannot, on its own, prove superiority over belzutifan or establish a market beyond kidney cancer.
Start small before the proof. Build the position as the evidence builds. And never forget the price.
Biotech Explained — Science first. Investment second.
Prepared October 18, 2026, before the new data presentation.
What Happened in the Biotech World Today?
$RHHBY $LLY $IONS $JNJ $ASND
October 16, 2026 — U.S. market close
Market temperature
S&P 500: 7,659.44 (−1.10%)
Nasdaq : 26,140.03 (−2.15%)
XBI: $144.32 (−3.65%)
SOXX: $493.39 (−4.14%).
SOXX closed lower for a ninth consecutive session, its longest losing streak since May 2006.
The VIX jumped roughly 25% to 20.57, reaching its highest level since late July.
Some investors hope the selloff is approaching a bottom. But neither a nine-day losing streak nor a rising VIX can tell us whether the low is already in.
Biotech was caught in the selling too: XBI more than gave back Thursday’s gain.
On the medical side, the day brought one U.S. approval and several European recommendations.
1. Roche / $RHHBY — Enspryng approved for thyroid eye disease in the U.S.
The FDA approved satralizumab for adults with thyroid eye disease. After training, patients or caregivers can administer this subcutaneous treatment at home.
In the Phase 3 SatraGO-2 trial, 52.9% of treated patients had a response in eye bulging, compared with 23.4% on placebo. Double vision improved in 60.7% versus 25.8%. SatraGO-1, however, missed its prespecified primary endpoint.
Why it matters: Patients gain another treatment option without routine infusion visits. The different outcomes of the two trials remain an important part of the evidence.
⸻
2. Lilly / $LLY — An oral GLP-1 recommended for approval in Europe
The EMA’s scientific committee recommended orforglipron, a once-daily tablet, for adults with obesity or type 2 diabetes.
In the weight-management studies reviewed by the agency, patients lost approximately 5% to 11% of body weight at 72 weeks, depending on dose, versus 2% to 2.5% with placebo.
Why it matters: Another oral GLP-1 could broaden treatment choices. A positive committee opinion is not yet EU approval; the European Commission must still decide.
⸻
3. Ionis / $IONS — Olezarsen advances in Europe for severe hypertriglyceridemia
The EMA committee recommended extending Tryngolza to adults with fasting triglycerides of at least 880 mg/dL (10 mmol/L), alongside diet and other lipid-lowering treatments.
In the randomized CORE and CORE2 trials, triglycerides fell by up to 72% versus placebo at six months. A pooled analysis reported 85% fewer acute pancreatitis events over 12 months.
Why it matters: Preventing pancreatitis is the clinical goal behind lowering these very high triglyceride levels. The proposed European extension still awaits the Commission’s decision.
⸻
4. Johnson & Johnson / $JNJ — Talvey could move earlier in multiple myeloma treatment
The EMA committee recommended talquetamab plus daratumumab, with or without pomalidomide, for certain adults with relapsed or refractory multiple myeloma after at least one prior therapy.
In the randomized Phase 3 MonumenTAL-3 trial, both combinations reduced the risk of progression or death compared with the control regimen. Adverse effects, including infections, remain an important part of treatment decisions.
Why it matters: This could bring a bispecific immunotherapy into an earlier line of care. The expanded indication is not yet authorized in the EU.
⸻
5. Ascendis / $ASND — A weekly achondroplasia treatment moves closer to Europe
The EMA committee issued a positive opinion for navepegritide in children aged two or older with achondroplasia whose growth plates remain open.
Why it matters: If the European Commission approves it, families would gain another option administered once a week. The committee opinion itself is not a marketing authorization.
⸻
Science to watch — Estuary Biotherapeutics
Estuary launched with $103 million to develop two locally delivered gene therapies targeting the PKD2 and PKD1 forms of autosomal dominant polycystic kidney disease. Its approach is scientifically interesting, but both programs are preclinical. Entry into human studies is planned for 2027 and 2028, respectively.
⸻
Advance of the day: Roche / $RHHBY.
Enspryng’s approval changes the options available to eligible U.S. patients now. The promise of at-home treatment is real, while the missed primary endpoint in one of its two Phase 3 trials deserves to stay in the story.
