The magnesium LOE was rated as high; but really 66% of patients came from a single study and most patients received digoxin (rather than CCB or BB). it may be an adjunct to use in difficult-to-control rates, but alone not a magic bullet with limited evidence

Nice to have an @ACEPNow clinical policy supporting the use of high-dose nitrates for acute heart failure. Nitro gtt + NIPPV = closet thing to magic we have for some of the sickest looking patients presenting to the ED 🪄 🧙♂️
