@EM_RESUS If the history is acute then aspirated FB is most likely. If symptoms are ongoing for longer then a malignancy, like a carcinoid is possible.

@EM_RESUS Abrupt end of the left main bronchus probably by aspirated foreign body or other endo-bronchial pathology that’s acting like a valve causing a collapse and consolidation of the left lung with mediastinal, diaphragmatic and chest wall pull. History will help DD.

@EM_RESUS Also it’s a NO for left pneumonectomy: it would leave a smaller bronchial stump and no lung markings.
