Where are we with Biomarkers in Rheumatoid Arthritis?
Biomarker interest has grown considerably in the last 2 decades, owing to advances in genetics, imaging, protein, and multiomics. Despite these advances, biomarkers as the predictive holy grail of RA therapeutics and prognostication have not yet advanced beyond rheumatoid serologies and CRP.

Prospective study of 6 newly Dx RA pts starting MTX shows MTX causes a decline in T follicular helper (Tfh) cells and plasmablasts (@ wk 3 before clin response), selectively in responders but not nonresponders. MTX shifts cellular & gene expression towards a more physiological state. https://t.co/tCfyCaGbiD
#TB Infection or reactivation is Psoriasis or Psoriatic Arthritis pts taking IL-23 inhibitors is rare, rare. Literature review showed 34 studies of IL-23i in PsO had no new TB, but 1 case of LTBI reactivation. In 8 IL-23i PsA studies there were NO new TB or LTBI reactivations https://t.co/SQGj6eZO9N
A Disease Seeking Its Identity: Peripheral SpA, PsA, and the SPARTACUS Evidence
For years, pSpA has been managed with a cautious, stepwise approach: start with NSAIDs, escalate to conventional synthetic DMARDs, and only reach for biologics when everything else fails. The SPARTACUS trial, presented at EULAR 2026 in London (OP0107), challenges that paradigm head-on. The results make a compelling case for rethinking when, not just whether, to deploy TNF inhibitors.

Single center study of 102 pts starting JAK inhibitor (72% >50yrs; 77% RA) shows only 27.5% received at least one RZV vaccination dose (Shingrix) & only 3 recv RZV within 30 daysof a JAK inhibitor. ????? (I vaccinate everyone going on a JAKi w/ Shingrix - don't you?) https://t.co/rA97gDttGs
Prevalence of GI involvement in dermatomyositis (DM) was assessed in 2 cohorts (n = 167). Severe GI involvement seen in 18% NXP2+ & 6% MDA5+; more common w/ NXP2 than other DM (HR 8.85). NXP2 w/ severe GI dz were younger, facial edema @dx w/ both skin & muscle dz activity. https://t.co/2m0huaDfZQ
How many CAR-T’s do we need to test drive?
There were 93 abstracts with CAR-T in title at #EULAR2026! Additionally, there was an excellent review (WIN) by Dr. Hector Chinoy. We know lots but have more questions.

From BiTE to TriTE: Progress of T-cell Engagers in RMDs
At EULAR 2026 in London, T-cell engagers drew attention as a strategy to achieve deep B-cell depletion and drug-free remission in refractory RMDs. BiTEs offer off-the-shelf availability, reversible toxicity from short half-life, no need for conditioning/lymphodepletion, and lower cost.

MTX: SC vs Oral? Metanalysis of 9 RCTs, 974 RA pts. Compared w/ oral, SC MTX signif. incr odds of ACR20 (RR=1.15), ACR50 (RR=1.14) & reduced GI adverse events (RR=0.58), diarrhea (RR=0.42): BUT no difference in ACR70, DAS28-ESR, bioavailability AUC, or other AE https://t.co/9yHhy2ibGW

Vasculitis Updates: Glucocorticoid use in AAV and GCA
Vasculitis Updates: Glucocorticoids are the cornerstone of initial therapy for AAV and GCA—quickly reducing inflammation and preserving organs. EULAR has issued recommendations on glucocorticoid use in ANCA‑associated and large‑vessel vasculitis. Evidence on long‑term use is evolving.
