@dominicgates For me this makes perfect sense. If the record to open the door was created, the task to inspect the bolts would have been generated. It is very believable that no paperwork was created, and this is (one of ) the causes of the escape.
@npert7@OnDisasters I think this is exactly the reason. People in aerospace have long memories. I would expect people at those sites that grew up as Shorts and Bombardier still think of themselves that way, and the site culture is mostly whatever it was before. Takes a long time to move that dial.
