Fascinating look inside an oral microbiome case study with @bristlehealth.
The patient walks away with a prescription for xylitol gum, probiotics, and some foods/spices to avoid or add, and now has a way to address persistent gut issues.

I've talked at length about saliva testing, and today I'm going over an oral microbiome case from start to finish (with consent), no holds barred.
This test turned out better than I ever could have anticipated in demonstrating the utility in a test like this, you'll soon see why.
Medical History:
- 27 year old female
- Medication: Birth Control (control yourselves)
- History of PCOS, irritable bowel syndrome, gallbladder removal in 2019
- Dental History: Braces as a teenager, never had a cavity
Before we begin, I want to explain a few things:
1) This test was used under my supervision, but leveraged the direct-to-consumer salivary diagnostic kit from @bristlehealth. There is nothing stopping you from buying this exact same kit if your dentist doesn't offer this service.
2) The patient is personally connected to me, hence the ability to gain her consent to share her case as my patients are unaware of this account.
3) She expressed interest in having a salivary analysis despite having ZERO dental issues.
Alright, so the test is fairly simple: take a swab of your saliva> place it into the container > mail it out > wait 2-3 weeks for your results which quantify the bacteria that colonize your mouth.
Overview of the results:
As you can see, this is pretty decent:
- Optimal microbial environment against cavities
- Presence of beneficial bacteria
- Optimal environment against bad breath
- Decent Nitric Oxide production
But take a deeper look and things start to get interesting.
Leaning a little high on P. intermedia which has been associated with gum disease, cognitive disorders, cardiovascular disease, and musculoskeletal disorders. This is certainly a concern. I'd classify this on the "important/not-urgent" side of the Covey matrix.
Now digging deeper into the systemic connection:
She has a serious presence of Campylobacter and Fusobacterium, which are all implicated in digestive issues. Referring back to her medical history, we can see in real time how the gut and mouth are intimately connected.
Fusobacterium nucleatum is increased significantly in IBS-D patients, and involved in the pathogenesis of IBS by causing microbial dysbiosis.
Campylobacter concisus is commonly present in the oral cavities of almost everyone, but patients with IBD have significantly higher prevalence of C. concisus in their intestinal and oral tissues compared to healthy controls.
It's also noted that enteric (gut) strains originate from the oral C. concisus strain, meaning the bacteria starts in the mouth and travels to the gut.
If you look closely, her percentile for C. concisus is pushing 70% indicating something is outside our comfort zone for colonization.
All it takes is a few bad actors to plant their feet in your mouth to wreak havoc. If you look at her overall microbiome, you'd be quick to say "oh wow you're doing great and your teeth are perfectly healthy"
However, in the interest of viewing the PERSON as a whole, something is wrong even if she has perfect teeth (she does).
So wrapping up:
- She is extremely low risk for cavities and bad breath
- Her risk of gum disease is there despite excellent hygiene and gum health, by virtue of P. intermedia having a strong count
- Her gut issues are showing in her saliva due to the alarming counts of Campylobacter and Fusobacterium
So what do we do for this patient?
She went into this knowing it was an exercise in discovery rather than treatment, but that doesn't mean we can't take measures to aim for improvement at her followup salivary test in 3 months.
What I'm recommending:
1) We certainly want her to keep up her good hygiene but that's hardly a change since she's already in the top 1% in that regard. We're going to add in some Xylitol gum to get that P. intermedia under control (high sensitivity to xylitol).
2) Incorporating a probiotic in the form of supplement and fermented foods, which have been shown to have efficacy in restoring balance.
"Probiotic bacteria, such as Lactobacillus, Bacillus, Escherichia coli Nissle, and Bifidobacterium species, have a great immunomodulatory capacity and exhibit antipathogenic effects through various molecular mechanisms."
The fermented foods need to be introduced gently as they can cause exacerbation of her IBD if done haphazardly.
Some would argue that she needs to be put on antibiotics, but that's not my call. The literature doesn't have a great protocol for permanently reducing the bacterial loads we're aiming for, and at best cause a transient decrease. If her gastroenterologist has different recommendations, it's out of my jurisdiction.
3) Cinnamon
Cinnamaldehyde, a phenylpropanoid that gives cinnamon its flavour and distinct smell, is effective at killing Campylobacter in vitro. In this case we will start incorporating it into food, and even incorporating it into a mouth rinse.
4) Garlic
It contains bioactive compound allicin, which possess powerful antibacterial properties.
5) Avoiding Fumaric Acid
Fumaric acid is a food additive that has been shown to increase the growth rate of Campylobacter Concisus
6) Increase fiber intake
Increasing fiber in the diet is effective at keeping Fusobacterium nucleatum at bay, and overall can lead to improved outcomes in patients with IBD.
7) Green/Black Tea
Green and black tea extracts, EGCG, and theaflavins decreased the adherence of F. nucleatum to oral epithelial cells and matrix proteins. Moreover, these tea components also attenuated F. nucleatum-mediated hemolysis and hydrogen sulfide production, two other virulence factors expressed by this bacterium. In summary, this study showed that tea polyphenols may be of interest for treating F. nucleatum-associated disorders.
Is this a comprehensive plan? No.
But they are a decent starting point into moving her into the right direction. When I have a patient like this that is relatively low risk for dental issues, and would otherwise be told "Great job! See you in 6 months" we have gained a lot of insight into the systemic connection of the mouth and gut.
She sees a gastroenterologist, and will be taking these results with her at her next visit and if he wants to collaborate I'd be more than willing. This test was not meant to provide me with the responsibility of treating her gut issues, it just happened to turn out that way. She knows my recommendations are limited in scope, and was happy to discuss possible adjuncts to try to get those numbers down. Ultimately, she will seek guidance from her specialist, as she should on a legal and ethical basis.
I hope this was insightful, and at the very least gave you an appreciation for what's possible with regards to learning more about your oral and overall health.
