A day ago, we had a case of stab injury in the neck with thyroid cartilage fracture and full thickness laryngeal tear. What unfolded is a beautiful collaboration between ENT team and Anesthesia team, in securing the airway and repairing it. We have: 1)Secured a tracheostomy in a spontaneously breathing patient 2)Performed Videolaryngoscopy mutiple times intraoperatively, to help surgeon identify the mucosal tears/active bleeders and understand the anatomy while approximating fracture ends 3)Used a cuffed ETT as a splint to support the cartilage, while using tracheostomy tube to electively ventilate the patient intraoperatively and postoperatively 4)Intermittently gave PPV through the ETT to help surgeon identify any residual defects It's definitely a bummer that I couldnt click any pictures standing at the head end, but it was one hell of an airway experience.
